Purpose The purpose of this study was to investigate effectiveness of orthopaedic manual physical therapy on restore of joint and function on acute total knee replacement (TKR) patients. Methods The participants of this study include 30 patients with total knee replacement who are receiving admission treatment in Prehabilitation center. All subjects were acute stage patients within 4weeks of surgery. Patients were randomly allocated to a conventional physical therapy group (CPT) and an orthopaedic manual physical therapy group (OMPT). Conventional physical therapy is consisted of cold therapy, electrical therapy, continuous passive motion (CPM) and group exercise program. OMPT group was received additional manual therapy used slow eccentric isotonic stretch technique (SEIS) of muscle energy technique (MET). After 2 weeks of intervention, the outcomes were measured. The primary outcomes were pain, active range of motion (ROM) of knee flexion and knee extension, swelling. The secondary outcomes were timed up and go test (TUG), step length, step speed. Results Pain, active ROM in flexion, reduction of swelling in the primary outcomes was improved in both groups. However, active ROM in knee extension was significantly increased in the only OMPT group. Also, decrease of swelling was greater in OMPT group than CPT group. TUG was improved in all groups, but other gait parameters were not significant. Conclusions Orthopaedic manual physical therapy is positively effectiveness on the recovery of early TKR patients. The SEIS technique is effective in reducing edema and improving ROM in early TKR patients. Therefore, we suggest that using the SEIS technique, which is a gentle manual therapy for early TRK patients, will help in the early recovery of TKR patients.
