본 연구는 교대 근무 간호사를 대상으로 근무 후 아로마 에센셜 오일 흡입을 통해 스트레스, 수면의 질, 면역에 대한 효과를 파악하기 위한 동등성 대조군 무작위 실험연구이다. 연구 대상은 2017년 5월1일부터 2017년 9월 15일까지 D시 소재에 E대학 병원 내. 외과 병동에 근무하는 3교대 근무 자로 4일 연속 낮 번 근무를 하는 간호사였다. 표본 크기는 G.Power Analysis를 근거로 선행연구의 Partial η2=0.06으로 효과크기를 계산하여 효과크기 0.252, 양측검정유의 수준 .05, 검정력 .95, 두 그룹으로 측정횟수 4, 반복측정 상관 계수 0.3, 비구형성 1로 표본 수를 산출한 결과 총 50명이 필요하였다. 최종 연구 대상자는 선정기준 및 제외기준을 만족하는 자로 아로마군 31명, 대조군 32명으로 하였다. 자료 수집 전 연구진행에 관해 E대학병원의 기관생명윤리심의원회(IRB)의 심의를 통과 하였으며, 아로마 특성상 시험처치를 알 수 있는 개방 수준(open label)으로 진행하였다. 아로마 에센셜 오일 흡입의 효과를 확인하기 위해 아로마군에게는 라벤더, 일랑일랑, 네롤리 오일을 블렌딩하여 아로마 스톤에 2방울 떨어뜨려 코 앞 10cm에서 3분 동안 심호흡하도록 한 후 베개 반경 30cm근처에 두고 수면하도록 하였다. 대조군에게는 아로마 에센셜 오일을 제공 하지 않고 수면을 하도록 하였다. 측정은 실험 처치 전(Do), 실험 처치 1일째(D1), 2일째(D2), 3일째(D3)총 4회에 주관적 스트레스, 수면의 질을 측정하였고, 실험처치 전(D0), 실험 처치 후 3일째(D3)총 2회에 혈청 코티졸(Cortisol)과 혈청 면역글로불린 A(Immunogloblin A, IgA) 측정하였다. 스트레스는 스트레스 점수(NRS, Numeric rating Scale)와 혈청 코티졸을 측정하였고, 수면의 질은 수면의 질 점수(NRS)와 수면의 질(VSH, Verran & Snyder-Halpern)도구, 입면 후 각성 횟수(NoA, Numbers of awakening)를 측정하는 Actigrph를 이용하여 실험 처치 전(Do), 실험 처치 1일째(D1), 2일째(D2), 3일째(D3)총 4회 측정하였다. 수집된 자료는 SPSS 24.0 통계 프로그램을 이용하여 분석하였다. 변수의 정규성 검정을 시행하였으며 그 결과에 따라 분석방법을 결정하였다. 두 군 간의 일반적 특성과 종속변수의 동질성 검정은 t-test와 χ2 test로 분석하였고, 아로마 에센셜 오일 흡입 처치 적용 전. 후의 차이 검증은 t-test와 반복측정 분산 분석(Repeated measures of ANOVA), 반복측정 공분산 분석(Repeated measures of ANCOVA)를 이용하여 분석하였다. The present study is parallel group ramdomized controlled trial aimed at determining the effects of the inhalation of aroma essential oils on the stress, quality of sleep, and immunity of shift work nurses. The participants included shift work nurses working day shifts for four consecutive days in medical and surgical wards at E university hospital in D city, from May 1 through September 15, 2017. The sample size was estimated by calculating the effect size with Partial η2=0.06 of previous studies. Based on G power analysis, the total number of participants required was 50 for the effect size of 0.252, α of .05, a power of .95, number of two group measurements of 4, the correlation coefficient of repeated measures of 0.3, and the non-sphericity of 1. The final number of participants who satisfied the inclusion and exclusion criteria were 31 for the aroma group and 32 for the control group. Approval was obtained from the Institutional Review Board (IRB) of E university hospital before data collection. The study was conducted in open label where participants are familiar with experimental treatments owing to the nature of aromatherapy. To determine the effects of inhalation of aroma essential oils, lavender, ylang-ylang, and neroli oils were blended and the aroma group was instructed to place two drops on an aroma stone, take deep breaths for three minutes with the aroma stone 10 cm away from the nose, and then sleep with the stone placed about 30 cm away from the pillow. The control group was instructed to sleep without the aid of the aromatherapy essential oils. Subjective stress and the quality of sleep were measured before the experimental treatment (D0), and at the first day (D1), second day (D2), and third day (D3) of experimental treatments, for a total of four measurements. Serum cortisol and serum immunoglobulin A (IgA) levels were measured at first day (D0) and third days (D3) the experimental treatments, for a total of two measurements. Stress was measured with stress scores on a Numeric Rating Scale and serum cortisol levels, and sleep quality was measured using sleep-quality scores (NRS) and a VSH scale (Verran & Snyder-Halpern). The number of awakenings (NoA) after falling asleep was measured using an actigraph at D0, D1, D2, and D3 of experimental treatments, for a total of four measurements. The collected data were analyzed using the SPSS 24.0 program. The normality test of the variables was tested and the analysis method was determined based on the result. Homogeneity test for general characteristics and dependent Variables in two groups were analyzed using t-test and χ2 test, while the differences between before and after the aroma essential oil inhalation treatment were analyzed using t-test, repeated measures of ANOVA, and repeated measures of ANCOVA. The Mauchly sphericity test was performed before repeated measures of ANOVA; and if sphericity was not satisfied, Wilks lambda in MANOVA was used. However, for the aroma group, sleep appeared to be induced within 30 minutes after the experimental treatment, while the induction stage of the control group still appeared to be 30 to 60 minutes of sleep on the first (D1), second (D2), and third (D3) day of the experimental treatment. Therefore, aroma essential oils are considered to be helpful for the improvement of sleep induction and sleep quality.
