Objectives: The purpose of this study is to evaluate the effects of cupping therapy on patients with migraine in randomized controlled trials through systematic review and meta-analysis.
Methods: We searched the studies in seven databases(EMBASE, MEDLINE, CENTRAL(The Cochrane Library), CNKI, KoreaMed, KMBASE, OASIS) and selected the relevant studies. Quality of the literature was assessed using the 'Risk of bias(RoB)' tool of the Cochrane collaboration, meta-analysis was performed using the Review Manager 5.3(The Cochrane Collaboration, 2014).
Results: The 7 studies were systematically reviewed. Meta-analysis was performed on 5 studies and quantitative evaluation was made on 2 studies. Total clinical effective rate of blood letting cupping therapy was 1.22(RR 1.22, 95% CI 1.08, 1.37) and the migraine score was 3.29(MD -3.29 95% CI: -8.22, 1.64) lower than western medicine therapy. The number of migraine episodes, headache intensity score, headache duration time score, and accompanying symptom scores were lower in blood letting cupping therapy than in western medicine therapy, but the difference was not statistically significant(p> 0.05). The adverse reactions were less in blood letting cupping therapy than in western medicine therapy. Total clinical effective rate of cupping therapy combined with acupuncture therapy was 1.05(RR 1.05, 95% CI: 0.99, 1.12) and VAS(headache) was 2.29(MD –2.29, 95% CI: -2.62, -1.96) lower than acupuncture therapy. The results of the quality evaluation showed that the risk of bias was generally high.
Conclusions: The results of systematic review and meta-analysis showed that cupping therapy improves the clinical treatment effect of migraine and significantly reduces headache intensity and accompanying symptoms related to migraine. In the 2 studies, it was showed that cupping therapy was a safe treatment because there was less adverse reaction than western medicine. In order to objectively assess the effect of cupping therapy, a well-designed randomized controlled trial with objective indicators will be needed in the future.
