Recently, measurement and analyses have been conducted using an inertia measurement unit (IMU) sensor that is worn by subjects; this provides quantitative evaluations of these parameters, and this test is referred to as the instrumented TUG (iTUG) test. This differs from prior studies because it provides an objective method to predict falls using the results of the iTUG test. In the present study, the reliability of the measured values was evaluated; in addition, a follow-up study was performed using the quantitative iTUG test to examine the characteristics of the fallers and non-fallers that are difficult to identify in a case-control study. A total of 69 elderly people participated in the study (faller: 26, non-faller: 43), and follow-ups were conducted at a specific time of the year for two years. Each experiment was repeated twice. A faller was defined as an individual who experienced a fall more than once in the past 3 years. In the TUG test, acceleration and gyro data were measured using the IMU sensor system (APDM Inc.). The measured data were divided into phases according to the TUG test operation, and a total of 48 linear and 12 non-linear variants were calculated. The test-retest reliability (repeated measure data in each year) of the intra-class correlation coefficient (ICC) was very low in the sit-to-stand phase. The difference in the variables between the faller and non-faller groups differed across each yearly experiment. Results of logistic regression analysis showed that included variables of the fall prediction model showed no correlation in the model for each year. Finally, the predicted accuracy of the fall model based on the entire follow-up data was 70.2%(without sit-to-stand: 69.1%). Variables in the sit-to-stand phase duration had limited use in fall risk assessment. Moreover, because of the limitations of the case-control study, the follow-up study was required for fall risk assessment. Follow-up data were used for analysis using a logistic regression model of fall prediction. The results of this study allow the further development of a more quantitative iTUG test design and a highly accurate fall prediction model.
