전자담배는 전자담배 액상을 열로 기화시켜 증기를 흡입하는 원리로 작동된다. 전자담배는 일반담배보다 안전하다는 이유로 많은 사람들이 사용하고 있으며, 그 시장은 계속해서 증가하고 있는 추세이다. 하지만, 여러 연구들을 통해서 전자담배가 간암, 심장질환, 구강암 등과 같은 다양한 질병을 야기한다는 보고가 이어지고 있다. 본 연구에서는 전자담배가 중이염에 미치는 영향을 연구하고자 한다. 현재 전자담배 회사에서 판매하고 있는 다양한 종류의 전자담배 액상을 이용하여 전자담배가 인간중이상피세포에서 세포사멸, 염증과 뮤신 발현, 세포 삼투압 이상 등을 분자생물학적 기법으로 확인하였다. 흥미롭게도 전자담배에 포함된 향의 종류에 따라 인간중이상피세포의 생존율이 다르게 나타났으며 염증과 뮤신 발현 등 중이염 관련 세포 반응도 다르게 분석되었다. 중이염 관련 유전자 발현 분석을 위해 전자담배를 처리하였을 때, 인간중이상피세포에서 염증 관련 COX-2와 TNF-α의 mRNA 발현이 증가하는 것을 확인하였고, Mucin 생산의 증가도 확인하였다. 뿐만 아니라 인간중이상피세포에서의 세포 내 물 수송 채널 관련 유전자들의 발현이 감소하여 세포 삼투압 조절 이상이 발생할 가능성을 확인하였다.
마지막으로 유세포분석기를 이용하여 apoptosis를 통한 세포 사멸 기작을 확인하였다. 이 결과들은 전자담배가 인간중이상피세포에서 유의적으로 세포사멸 유도와 염증 반응 촉진, 그리고 세포 내의 물 수송 채널의 조절장애를 유발하여 세포 내 액체 유입을 증가시킬 수 있음을 확인하였다. 그럼으로, 본 연구를 통하여 전자담배 액상 성분이 중이염을 야기할 수 있는 유해 물질이 될 수 있음을 밝혔다.

Electronic cigarette (e-cigarette) operates to vaporize electronic cigarette liquids (e-liquid) using heat by coil in atomizer and user inhales the aerosol. Since e-cigarette has been considered less harmful than conventional cigarette, many people have used e-cigarette and the markets have been growing continuously. However, many studies showed that e-cigarette caused various harmful diseases such as lung cancer, heart diseases and oral cancer. In this study, we studied the effect of e-cigarette on Otitis Media (OM). Results showed that e-cigarettes caused cytotoxicity via apoptosis, inflammatory response, and mucin production on human middle ear epithelial cells (HMEECs). Cell viability of HMEECs was different among flavors. When the cells were treated to e-liquids, the expression of COX-2 and TNF-α were stimulated and mucin production was increased. In addition, e-liquids stimulated dysregulation of water channels in HMEECs and it could cause fluid accumulation. Finally, apoptosis signaling was induced by exposure to e-liquids in HMEECs, which was analyzed by flow cytometry. These results showed that e-cigarette increased inflammatory response and caused dysregulation of water channels to accumulate of fluid in HMEECs. It implies that e-cigarette liquid can be a risk factor for OM.
