This study will examine the relationship between chronic diseases and unmet medical experiences to provide basic data to establish healthcare policy to improve present situation. This study was based on Korean nationwide survey of elderly population over 65 years old of 2014 and 2017. We analyzed 20,325 out of 20,750 excluding missing data as well as insomnia and Parkinson’s diseases which were newly added in 2017. Univariate analysis was performed with Rao-Scott chi-square method on socio-population, health-function and chronic diseases factors according to sex. Regression models were built by survey logistic regression; first step without adjustment; second step adjusting socio-population factors; third step adjusting health-function factors. Statistical analysis was performed with SAS version 9.4. This study revealed 8.7% of study population, 1,729 out of 20,325 had unmet medical experiences. It showed sexual difference of male 6.2% and female 10.1%(p<.001). Comparing to control male without chronic diseases, third step regression model showed respiratory diseases had low unmet medical experiences with odds ratio of 0.43(95%CI=0.22-0.86) and cataract had high unmet medical experiences with odds ratio of 1.70(95%CI=1.19-2.43). For female population regression model showed low unmet medical experiences were assoicated with hypertension with odds ratio of 0.79(95%CI=0.67-0.92), cancer with odds ratio of 0.58(95%CI=0.38-0.90) and chronic kidney diseases with odds ratio of 0.45(95%CI=0.23-0.86). It showed high unmet medical experiences were associated significantly with osteoarthritis or rheumatoid arthritis with odds ratio of 1.51(95%CI=1.29-1.77), cataract with odds ratio of 1.34(95%CI=1.09-1.66) and gastro-duodenal ulcer with odds ratio of 1.29(95%CI=1.03-1.60). There were differences in chronic diseases associated with unmet medical experiences over sex. For both sexes unmet medical experiences were associated significantly with marriage, education, household, children-relationship, region, annual income, economic activity, house-ownership, activities of daily living, fall-down, nutrition, depressive mood and subjective health status. This study showed relationship between various chronic diseases and unmet medical experiences among a large representative elderly population over 65 year old over sex adjusting confounding variables such as socio-population and health-function factors. Different diseases revealed different unmet medical experiences. With extensive governmental investment on healthcare insurance, four major serious categories showed low unmet medical experiences. But diseases associated with natural aging that elderly people might not think them as diseases showed high unmet medical experiences. Among them were osteoarthritis, rheumatoid arthritis, cataract and gastro-duodenal ulcer. More detailed research might be necessary for causes of those high unmet medical experiences. Practice guidelines are to be developed for appropriate practice for elderly chronic diseases. Policies are to be improved for awareness of elderly chronic diseases and their control. We anticipate appropriate control of elderly chronic diseases would help healthier lives of elderly population and preventing unnecessary medical expenditure.
