This study was conducted to investigate the characteristics of complication patients by tracking the occurrence of complications in patients with hypertension. This study used the National Health Insurance Service-National Sample Cohort from 2002 to 2014. Among 1,025,340 people (513,258 males and 512,052 females) accounting for approximately 2.2% of the total population of 46,605,433 in 2002, 448,066 people who were in their 30s and had a qualification database, a disease database, and a billing statement database were randomly selected. Out of them, 272,748 people were selected as the subjects, excluding those who had high blood pressure and complications in 2002 and 2003. From the subjects selected, 12,675 people who had essential (primary) hypertension in either the principal diagnosis or the secondary diagnosis in their medical database were finally selected as the research subjects. The results of the study showed that, in terms of the demographic characteristics of complications of each year, complications occurred more frequently in males and in their 70s or older. Complications occurred more frequently in the households of lower income quintile and in counties and small cities. The annual incidence rate of complications was about 4% to 5%. As for the distribution of diseases in patients with complications, cerebral infarction, which belongs to cerebrovascular diseases, as well as atrial fibrillation and flutter and acute myocardial infarction, which belong to cardiovascular diseases, occurred frequently, and heart failure occurred the least with the exception of cardiac arrest. In terms of disease-specific distribution with regard to the incidence order of complications, the first complication occurred in the order of cerebral infarction, atrial fibrillation and flutter, and acute myocardial infarction. However, the second complication occurred in the order of cerebral infarction, acute myocardial infarction, and atrial fibrillation and flutter. In the disease-specific distribution by demographic characteristics, cerebral infarction occurred frequently in all ages, all the range of income quintile, and all regions, followed by acute myocardial infarction, and atrial fibrillation and flutter with some difference in the order by the characteristics.
