Introduction The association between shift work and sleep disorder is well known. However, there have been few researches on how sleep disorders vary from occupation to occupation. Shiftwork is a known risk factor and trigger for depression. The aims of this study were to investigate sleep disorder prevalence and patterns in shift workers in various occupational groups and the factors affecting sleep disorders with or without depression.
Materials and Methods A search of the nationwide night-shift Workers’ Specific Health Examination (nsWSHE) data collected in 2015 identified that the most common type of night-shift work was in the manufacturing industry, followed by the human health and social welfare industry, the business facilities management and business support services industry, and transportation and storage industry. A survey was also conducted in 214 apartment workers, 295 hospital workers, and 222 taxi drivers. The crude odds ratios and adjusted odds ratios were conducted after adjusting for sex, age, marital status, education level, cigarette smoking, alcohol consumption, opportunities for night-time naps, occupation (hospital workers, apartment workers, and taxi drivers), and depression and fatigue. The relationship between these potential sleep disorder-associated factors and depression was investigated further by stratification analysis of depression and above factors associated with sleep disorder was conducted. Group interviews were also conducted to identify work conditions that could increase the risk of a sleep disorder.
Results The total prevalence of sleep disorders was 4.6%, 5.7% in the human health and social welfare industry, 3.9% in the business facility management and business support services industry, and 3.6% in transportation and storage industry. Sleep disorders were significantly associated with sex, age, and workplace size in all three occupational groups. The factors associated with a sleep disorder was symptoms of depression (odds ratio [OR] 3.249, 95% confidence interval [CI] 1.938-5.446), fatigue (OR 3.545, 95% CI 2.083-6.032), and more than college graduation (OR 2.396, 95% CI 1.220-4.707). There were differences in the factors associated with sleep disorders of workers with and without depression. In the group without depression, there was a significant association of more than college graduation (OR 4.429, 95% CI 1.736-11.304), and opportunity for a night-time nap (OR 0.308, 95% CI 0.116-0.821) with sleep disorders. In the group of depression, there was a significant association of smoking with sleep disorders (OR 10.373, 95% CI 1.567-68.673).
Conclusion This study of data for night-shift workers who participated in the WSHE is the first to analyze the prevalence of sleep disorders in Korea. The findings of this study can be used to prioritize intervention policies for sleep disorders in night-shift workers.
