결과 : 빈혈의 발생 비율은 저체중군 중 미달군(OR 3.68, 95% CI 2.09~6.48)및 초과군(OR 7.09, 95% CI 1.23∼40.89), 정상체중군에서 미달군(OR 1.56, 95%CI 1.03~2.36)에서 의미 있게 증가하였다. (OR 3.28, 95% CI 1.54~7.00; OR 8.94, 95% CI 2.54∼31.41; OR 3.58, 95% CI 1.22∼10.56)에서 의미 있게 비율이 증가하였다. (OR 7.86, 95% CI 1.25∼49.41; OR 2.75, 95% CI 1.11∼6.80; OR 4.40, 95% CI 1.29∼14.96)에서 유의하게 높은 발생 비율로 나타났다.

Purpose : To analyze the pregnancy outcome and prognosis related to prepregnancy Body mass index (BMI) and gestational weight gain in twin pregnancies according to Institute of Medicine recommendation (2009).
Materials and Methods : The study subjects were 500 twin-pregnant women and their 1000 babies delivered in CHA Gangnam Medical Center from January 2008 to December 2010 and the study was done retrospectively. The pregnancy outcome and perinatal complication was analyzed according to prepregnancy maternal BMI and gestational weight gain. The maternal BMI was divided 4 groups according to WHO Asia-Pacific guidelines (2004) in this study; Underweight (BMI<18.5 kg/m2), Normal weight (18.5≤BMI<23 kg/m2), Overweight (23.0≤BMI<25 kg/m2) and Obese (BMI≥25 kg/m2). Gestational weight gain was categorized as "Poor" if it was below the IOM recommendation range for the prepregnancy BMI, "Normal" if it was within the range, and "Excessive" if it was above the range.
Results : Anemia was significantly increased in "Poor" and "Excessive" gestational weight gain group with underweight ("Poor", odds ratio (OR) 3.68, 95% CI (Confidence Interval) 2.09~6.48; "Excessive", OR 7.09, 95% CI 1.23∼40.89) and "Poor" gestational weight gain group with normal weight (OR 1.56, 95%CI 1.03~2.36). Gestational hypertension was significantly increased in "Excessive" weight gain group with normal weight, overweight, Obese BMI (normal weight, OR 3.28, 95% CI 1.54~7.00; overweight, OR 8.94, 95% CI 2.54∼31.41; obese, OR 3.58, 95% CI 1.22∼10.56). Gestational diabetes mellitus was significantly increased in Obese group ("Poor", OR 5.02, 95% CI 1.83∼13.73; "Normal" OR 4.40, 95% CI 1.48∼13.07). Neonatal Apgar score of 5 min under 7 was significantly increased in "Excessive" weight gain group with normal weight (OR 3.89, 95% CI 1.06∼14.3) and obese BMI (OR 11.72 95% CI 2.65∼51.82). Small for gestational age was significantly increased in "Poor" weight gain group (underweight , OR 2.44, 95% CI 0.94∼6.33; normal weight OR2.21, 95% CI 1.08∼4.54). Large for gestational age was significantly increased in "Excessive" weight gain group (underweight, OR 7.86, 95% CI 1.25∼49.41; normal weight, OR 2.75, 95% CI 1.11∼6.80; obese OR 4.40, 95% CI 1.29∼14.96).
Conclusion : The adverse pregnancy and neonatal prognosis were associated with prepregnancy underweight, overweight, or obese BMI with poor or excessive gestational weight gain in twin pregnancies. Therefore, it is important to maintain normal prepregnancy BMI and gestational weight gain through appropriate counseling and education. A large prospective study is necessary to confirm the relationships between gestational weight gain and pregnancy outcome in twin pregnancies.
