Purpose: This study is to contribute to a positive birth result and proper prenatal care understanding that maternal prepregnancy body mass index (BMI) and gestational weight gain affect maternal and neonatal outcomes. Method: Study subjects were 1910 pregnant women who delivered their baby with gestational age from 37 to 42 weeks at a university hospital in Busan from 2004 to 2013. Collected data were maternal characteristics (age, prepregnancy weight, prepregnancy BMI, gestaional weight gain), obstetrical characteristics (parity, delivery type, induction delivery, complication during pregnancy, complication after delivery, period of in hospital), and neonatal characteristics (gender, weight, Apgar score, period of in intensive care unit). The outcomes by prepregnancy BMI and gestational weight gain compared with chi-square test and odds ratio (95% CI) using MedCalc (ver 14.10.2., Mariakerke, Belgium). Result: By prepregnancy BMI, the higher BMI group showed higher complication in pregnancy (OR=1.93, 95% CI=1.17~3.17), complication after delivery (OR=1.45, 95% CI=0.90~2.33), caesarean section (OR=1.81, 95% CI=1.35~2.43), LGA (large for gestational age) (OR=2.05, 95% CI=1.13~3.74) and NICU admission (OR=2.29, 95% CI=1.58~3.31) than the normal BMI group. By gestational weight gain, over weight gain group showed higher induced delivery (OR=1.29, 95% CI=1.03~1.62), LGA (OR=2.50, 95% CI=1.39~4.50), NICU admission (OR=1.49, 95% CI=1.57~2.19) than recommended gain group. Conclusion: The study shows that prepregnancy BMI and gestational weight gain affects maternal and neonate outcomes. Therefore it is required that active education and nutritional understanding for proper weight control during pregnancy to be implemented.
