본 연구에서는 정신장애인을 효과적으로 관리할 수 있는 ACT(Assertive Community Treatment) 프로그램을 정신건강토탈케어서비스에 적용해 보고 그 효과를 검증하였다. 중증 정신장애가 있는 사람에게 대상자의 증상 및 기능 수준, 사회생활 기술능력, 삶의 만족도 및 가족부담 등의 변화를 실증적으로 분석하였다. 이를 위해 지역사회서비스 정신건강토탈케어서비스를 이용하는 정신장애인을 대상으로 2017년 3월부터 2019년 2월까지 약 24개월 동안 추적한 결과이다. In this study, the ACT program to effectively manage the mentally disabled was applied to the mental health by Total Care Service and its effectiveness was verified. Changes of symptoms and functional levels of subjects, social life skills, life satisfaction and family burden were analyzed empirically in people with severe mental disorders. The study were maintained for about 24 months from March 2017 to February 2019, targeting mentally disabled people who use local investment social services mental health total care services. ACT (Assistant Community Treatment) is an active community care model, where wide ranges of services are served 24 hours a day, directly by the case team, in the place where the target is located. Mental health total care service is a case management services that provide counseling, crisis intervention, symptom management and support for daily life in order to prevent hospitalization and adapt to the community through early detection and intervention (life management) of the mentally disabled. The measures included the simplified psychopathological assessment scale (BPRS) that evaluates mental and mental symptoms of people with chronic mental disorders, the (SSI) that evaluates thoughts of suicide, the independent life function assessment index (ILSS) that assesses the independent living functions of people with mental disabilities in residential facilities, and the Stress Response Scale (CISS) that evaluates the stress response of people with mental disabilities. The main results were as follows; First, that the psychiatric symptoms (entire) and sub-scale withdrawal/delay, anxiety/depression, hostility/doubt/excitement, etc. were reduced post-evaluation compared to the preceding evluationd, and that accident disorder was not reduced. Second, the number of suicides in this case management group has decreased compared to the previous one. Third, the independent living function (full) scores and sub-scale of this case management group have decreased postmortem compared to the prior period, including personal hygiene, grooming, housekeeping and cleaning, communication, leisure activities, drug use and health care, sleep and excretion. Fourth, during the stress response of this case management group, task-oriented and social support pursuit increased posthumously compared to the dictionary, but emotion orientation decreased. The subjects of this study were given case management services, which led to more rational and proper pursuit of interpersonal relationships, and there were positive changes in the reduction of irrational emotional responses. These results proved that the ACT program worked in local contexts. The limitations of this study and the future direction of the study were discussed.
