아토피성 피부염(Atopic dermatitis)은 천식이나 알레르기성 비염을 동반하는 경우가 많은 만성재발성 피부질환으로 미국에서는 10~20%의 인구에서 발생하는 것으로 알려져 있으며, 국내에서도 전인구의 0.5~1%, 어린이의 5~10%가 아토피성 피부염을 앓고 있다. 이러한 아토피 피부염을 peripheral 관용유도, 이식 관용, 자가면역, 종양 억제, 모체-아기간 면역 관용에 작용한다고 보고되고 있는 Mesenchymal stem cell (MSC)의 면역억제기능을 이용하여 치료하고자 하였다. 우리의 연구에서 아토피 동물모델로 주로 사용되는 NC/Nga 생쥐를 사용하여 아토피 동물모델을 확립하고 아토피의 중증도를 측정하기위한 IgE의 농도를 측정하는 방법을 이용하여 아토피 유발의 유무를 확인 후 PBS를 처리한 그룹과 MSC를 처리한 그룹으로 나누어 동종의 쥐에서 분리 배양한 MSC는 MSC 처리 그룹의 아토피가 유발된 환부에 5 x 105cell을 1주일에 한번 총 4회 주입하여 실험하였다. 그 결과 아토피를 유발한 동물모델에서 아토피의 증상을 확인하였다. 그리고 동종의 동물모델의 지방으로 MSC의 대량배양을 확보하였다. 이렇게 아토피가 유발된 NC/Nga 생쥐의 IgE 농도를 측정한 결과 MSC를 처리한 그룹이 PBS를 처리한 그룹보다 높은 것을 확인하였다. 뿐만 아니라 아토피가 유발된 생쥐의 환부에 MSC로 처리한 그룹은 비만세포의 농도 또한 PBS를 처리한 그룹보다 높은 것을 Toluidin Blue O를 이용하여 확인하였다. 이로써 MSC는 아토피를 완화시키는 것이 아니라 악화시키는 것으로 보이며 지며 이는 MSC가 in vivo에서 Th2로 면역기능을 변하게 함을 예측할 수 있었고 이를 Th1와 관련된 질병의 치료에 활용할 수 있을 것으로 생각되며 우리의 MSC의 면역 기전 연구가 아토피의 치료에는 적합하지 않고 오히려 Th1에 의한 질환인 관절염과 같은 질환의 치료에 더 적합할 것이라 생각된다.

In this study we tested a hypothesis that syngeneic MSCs may suppress atopic symptoms established in murine NC/Nga mice which are atopy prone. An atopy model was successfully developed using NC/Nga mice and monitored by measuring the serum IgE concentration and mast cell deposition in the skin lesion. Therapeutic MSCs were isolated from adipose tissue of NC/Nga mice and cultured in vitro for MSC treatment (5 x 105 cells/mouse/injection x 4 injection/mouse). In vitro culture conditions to prepare a large amount of MSC were tested and mechanism of escaping or delaying senescence of MSC was identified. Injection protocols of MSCs into the atopic lesion we have tested were not a proper one for curing the atopy, instead atopic measurements including concentration of IgE and mast cell level in the lesion were significantly enhanced by the treatment. We concluded that the injection of MSC made worse the symptoms of atopy. Our data showed that Th1 responses might be decreasing while Th2 responses appear to be increasing by the treatment of MSC. The injection of MSC might be a good treatment model for that Th1 dominating diseases such as arthritis and allogeneic rejections.
