This study conceptualized suicide as a decision-making problem and explored the main judgment factors that involved in the decision to die and the characteristics of cognitive and decision-making behavior among those who choose to commit suicide. Study 1 presents the life and death choice situation through the euthanasia scenario and identifies the relationship among three decision-making variables: past life satisfaction, current pain severity, and prediction of future improvement. In addition, the effects of three variables on euthanasia choice were examined. Data were collected from 515 adults, who rated Suicidal Behavior Questionnaire-Revised(SBQ-R), Center for Epidemiologic Studies Scale-Depression(CES-D), Life Orientation Test-Revised(LOT-R), and Positive Affect and Negative Affect Schedule(PANAS), and one of four types of euthanasia scenarios. In all scenario types, the prediction of future improvement was strongly correlated with euthanasia choice, and the result of hierarchical regression analysis revealed that the future prediction was the most important variable explaining euthanasia choice. Current pain severity was the second most important variable explaining the euthanasia choice. In all types of scenarios, past life satisfaction was not a significant variable in predicting euthanasia choices. In Study 2, the performance of Future Thinking Task(FTT) and Iowa Gambling Task(IGT) of 21 suicide attempters, 22 healthy controls and 24 clinical controls were compared. Suicide attempters showed an reduced positive future thinking compared to clinical and healthy controls. In contrast, negative future thinking did not differ among three groups. Even when controlling for depression, the group effect of positive future thinking was significant. For the IGT total net score, clinical controls showed a significant decrease in the number of advantageous card selection compared to healthy controls. Although the suicide attempter showed similar performance to the clinical controls, the total net score difference from healthy controls was not significant. The result of repeated measure ANOVA revealed that only healthy controls showed increase in preferences for advantageous card deck toward the end of the block. The choice pattern of three groups were compared based on the ORL model, which proved to be the best model for explaining the card selection of three groups. Suicide attempters had lower sensitivity to reward and punishment than healthy controls. On the other hand, the discount rate of past choices was higher among the suicide attempters than among the healthy controls, which suggests that suicide attempters tended to forget past choice quickly regardless of whether the result of the previous choice was positive or negative.
