This study set out to investigate the effectiveness of integrated case management based on practical changes to and effects on case management users by applying the goal attainment scaling. For this purpose, the investigator found six integrated case management users for the experiment and control group each. The experimental group set individual goals using the goal attainment scaling, and applied the evaluation of the individual goals over the eighth round. The experimental group supported case management services, set and evaluate individual goals on their own, and continuously provided counseling interventions for each case. Also, the self-efficacy score was checked by the pre-post test. The control group only supported case management services and conducted a pre-post-test on the self-efficacy scale to see if there were any changes. Participants in the experimental group showed almost no change in goal attainment scores, but the application of the goal attainment scaling showed a positive result of self-efficacy. There were significant effects on social efficacy, self-acceptance, and self-achievement. In particular, self-acceptance in the experimental group was more significant than the control group.
The implications of this study are as follows. First, although the goal attainment scaling is used as an evaluative tool for clients in the field of social welfare practice, it can be used as a tool to intervene in the method of participating in the process of setting and implementing individual goals. Integrated case management establishes short-term and long-term goals with clients through assessments that reflect client needs. In this process, the goal attainment scaling can be usefully used as an intervention for the client's detailed problem solving. Categorizing your goals into five stages can help you recognize and work with a detailed process of action. Second, the practice process can include the process of self-reflection of the client. Among the sub-factors of self-efficacy in the experimental group, self-acceptance was found to be more significant than the control group. As clients practice their own problems, they first reflect on themselves. There is a process of understanding what one's desires and problems are and seeing them at the center before solving them. It can be interpreted that the self-acceptance can be formed to accept the self first. Third, the client's specific goal makes it easy to identify the client's situation and cause of the problem. It was found that most users of integrated case management who had various and complex problems perceived as having poor physical health and intended to set goals as the will to overcome them in the process of implementation. Due to various problems and environmental conditions of the clients, it is often difficult to take care of physical health, which leads to a vicious cycle in which physical health becomes worse. Fourth, integrated case management service support can influence the participants' individual goals. The difference between the experimental group and the control group on the self-efficacy was not significant, which indicates that the support of the integrated case management service had a significant effect on the two groups. In addition, when the scores of the experimental group were evaluated as falling in the process of changing the target achievement scores, the effect of increasing the target achievement scores through the integrated case management service was confirmed through the individual cases. Fifth, evaluation using the goal attainment scaling can be a useful tool for both the client and the case manager in the process of intervention and practice. Clients use the goal attainment scaling to reflect on their problems and situations by implementing their goals. During the evaluation period, this study was useful for helping students to check their own performance, their success and failure through scores, and to think about the causes. The case manager is a measure of goal achievement, but it was able to understand the client's problems and situations and help them set goals by being involved in setting the client's required goals. By consulting the client's goals with the scores, the client and the case manager were able to check the overall implementation process and explore future plans.
The suggestions through this study are as follows. First, since goal attainment scaling was developed as a tool for evaluating the effectiveness of clinical practice, in this study, goal achievement scale is useful for quantitative evaluation and client-specific evaluation in the effectiveness evaluation of integrated case management. I hope it can be used more. Second, since integrated case management has been expanded nationwide through the social welfare integrated network's computerized system, it is possible to reflect client's individual goal setting and achievement evaluation, so that overall evaluation that reflects client's opinion as well as case manager's evaluation can be achieved. I would like to suggest that improvement is needed. Third, this study briefly introduced the meaning of integrated case management of clients as consultation was conducted with case managers to evaluate individual management of case management users. Although this study attempted to prove its effectiveness as a quantitative study on integrated case management, it is hoped that subsequent research will provide an in-depth approach to the effects and implications of integrated case management on clients.
