Therapeutic application of extracorporeal membrane oxygenation (ECMO) in severe traumatic patients with traumatic lung injury and posttraumatic acute respiratory distress syndrome (ARDS) is challenging due to a high risk of bleeding and limited experience of medical practitioners. We investigated the clinical outcomes of 42 patients with traumatic ARDS who underwent ECMO. Traumatic ARDS cases included near-drowning (42.9%), traffic accidents (21.4%), crush injury (11.9%), fall down (9.5%), stabbing (4.8%) or gunshot (2.4%) wounds to the chest, as well as intoxication-induced pneumonitis or hanging (each 2.4%). Out of these, 34 patients were discharged with an average of 23 hospital days, and showed lower vasoactive inotropic score (VIS), simplified acute physiology (SAPSII ) scores, lactate level at pre-ECMO, higher platelet concentrate and pH in ABGA, and additionally required less transfusions than non-survivors. Multivariate analysis showed that lactate levels lower than 10.55 mmol/L and venovenous (VV)-ECMO were independent favorable risk factors for survival in patients with traumatic ARDS who underwent ECMO. In conclusion, ECMO demonstrated an excellent survival rates of 81% in traumatic ARDS patients, especially in those treated by VV-ECMO and showing low lactate levels (<10.55 mmol/L) at pre-ECMO.
