Human phantoms for dose evaluation are generally designed to be based on the European physique and characteristics. The purpose of this study was to investigate a Korean standard model of adult males and females and develop a prototype of Korean adult lung phantom using radiological tissue-equivalent materials. Body size data was collected from the seventh human body survey (2015, the National Institute of Standards and Technology report), and 2195 males and 2293 females aged between 20 and 60 years were statistically analyzed to calculate the average physical dimensions. Two hundred datasets of computed tomography corresponding to the average physique range were collected to measure the average linear dimensions (the length of x, y, and z-axes) of the lungs. With a retrospective study, the use of medical images stored in the PACS system for organ size analysis can collect thousands of human samples to determine the anatomical relationships between internal organs. Length, width, and height are parameters used to calculate the dimensions of an object. One image set of each male and female was finally selected as a sample model, which was the nearest to the average measured value. An image synthesis method is a new method to produce two-dimensional (2D) CT images into three-dimensional (3D) models without a CT scan of humans. The synthesis of the neck bone was carried out to examine the linear dimension and shape compared with the original non-synthesized model. The CT image analysis of neck bone models showed that synthesized models showed the difference was within ± 3% in length and ± 2%. Tissue-equivalent materials for lung and bone were verified the physical properties using a modification recipe controlled by adding the proper ratio of additives. Lung prototype phantom was constructed using tissue-equivalent materials after material tests. Structural evaluation and accuracy of the tissue-equivalency of the phantom were assessed using a CT scan with 80, 100, and 120 kVp in chest protocol. Phantom CT Images obtained were compared with the CT image of the human (sample) or LLNL phantom. The prototype phantom had a difference of 1% to 1.9% in length parameters, whereas the LLNL phantom had 10% in APL compared with humans. The deviation of CT number was within 2% of the four ROI except for one ROI at the right lung (19%). The dose measurement using EBT3 film was carried out to examine the significance of tissue equivalents on the specific geometric patterns of organs. The irradiated EBT films in this study applied the calibration curve for the conversion of pixel value and dose. The dose distribution of the prototype phantom was obtained by 10 MV photons in the linear accelerator (LINAC), and the level of consistency between the measurements and calculations was out of tolerance of 3% of the dose difference. When the dose was measured at the level of 15cm of the prototype phantom, the right lung was calculated 200 cGy and heart was 170cGy in the case of EBT film, the dose measured in the RTP was 200 cGy in the right lung and 150 cGy in the heart. There is 10~13% difference between EBT film and the RTP due to the irregular slice surface of the prototype phantom. However, the border of the lung-heart and muscle-lung area showed a steep variation of pixel values(more than 4000) due to radiation attenuation. The verification result shows that the prototype lung phantom accurately matches the proper application of the phantom study, which would result in high quality of phantom construction.
