Background: Dermal fillers made up of various biomaterials gain a lot of interests because of their simple and short surgical procedure while their effects are shown immediately after injections. Hyaluronic acid filler is the most popular for correcting wrinkles, providing facial volume and augmenting facial features, but it is known that some of hyaluronic acid fillers can cause side effects including itching, pain, redness, and edema. In order to minimize side effects while keeping durability, two different manufacturing methods were developed: Monophasic and Biphasic. Objective: To compare chemical properties, in vivo durability and safety of commercially available monophasic and biphasic hyaluronic acid filler. Methods: Chemical characteristics of both fillers were measured by NMR and SEC / MS. Both fillers were injected subcutaneously in mice to measure in vivo durability for 24 weeks and the safety of each filler was evaluated via histological analysis. Results: There was a significant difference of chemical characteristic between monophasic and biphasic hyaluronic acid fillers. Biphasic hyaluronic acid filler was confirmed to show higher value of degree of modification compared to that of monophasic filler. In addition, we also found that there was a difference on duration but found no safety difference between monophasic and biphasic hyaluronic acid filler. Histological studies showed that there was no sign of inflammation and foreign body reactions of both fillers at every time point. Conclusion: These findings suggested that monophasic and biphasic hyaluronic acid fillers have different chemical characteristics, but both fillers’ in vivo safety was not affected by the degree of modification. This study is the first research to compare chemical characteristic, in vivo durability and safety of monophasic and biphasic filler.
