This paper explores the role of Metacognitive Beliefs in the context of health anxiety in middle-aged women. In Study I, a tool to measure metacognitive beliefs in the context of health anxiety was validated and named The Korean version of Metacognitive belief – Health Anxiety (K-MCQ-HA). The K-MCQ-HA consisted of 14 questions and 3 factors; : “Beliefs that Thoughts can cause Illness”, “Beliefs about Biased thinking”, and “Beliefs that Thoughts are Uncontrollable” as same as the original scale. In order to evaluate the suitability of the K-MCQ-HA structure, a confirmatory factor analysis was performed, and various psychological characteristics and correlations related to metacognitive belief were analyzed. Confirmatory factor analysis supported the three factor model with all selected goodness-of-fit statistics as in exploratory factor analysis. K-MCQ-HA positively correlated with health anxiety, illness attitude, anxiety sensitivity, somatization, anxiety sensitivity and depression, while negatively correlated with subjective well-being and self-esteem. In particular, the K-MCQ-HA incremental validity was examined. The result indicated the MCQ-HA accounted for additional variance in health anxiety over and above that accounted for by Illness attitude which is known to measure the more generic cognition in health anxiety. These results prove the K-MCQ-HA has good internal-consistency, incremental, convergent and discriminant validity in relation to associated measures. The K-MCQ-HA captures more specific metacognitive content in the context of health anxiety and appears to be a potentially useful predictor of health anxiety. In Study II, in the process of physical self-concept and negative affect influencing health anxiety in middle-aged women, the moderated mediating effect of somatosensory amplification and metacognitive beliefs was verified. And the moderated mediating effect of metacognitive beliefs was examined in the high and low health anxiety groups. The results showed that metacognitive beliefs moderated the path from physical self-concept and negative affectivity to health anxiety through somatosensory amplification. In other words, in the case of middle-aged women who think they are weak or experience strong negative emotions, health anxiety may increase through somatosensory amplification and this process worsens if they have metacognitive beliefs. Furthermore, these results were only seen in high-health groups and not in low-health groups. This study show that somatosensory amplification mediates the relationship between physical self-concept, negative affect and health anxiety. Consistent with metacognitive theory, this association is also moderated by unhelpful beliefs about health and disease. High maladaptive metacognitive beliefs are likely to render the individual more sensitive to engage in difficult to control extended processing in response to negative interpretations of symptoms. This is a important finding that is in line with the metacognitive (S-REF) model. In this model the content of thoughts are more likely to cause psychological disorder because of their relationship with metacognitive beliefs. Therefore Metacognitive beliefs could become new therapeutic targets to help improve psychological treatments for health anxiety-relate distress.
