With the development of next generation sequencing technology, metagenomics research that can analyze genomes directly from environmental samples has been in earnest. Therefore, it was possible to study microbial communities existing in the environment by extracting DNA directly from environmental samples without separating or culturing microorganisms. With the advancement of this technology, we have learned how many types of microbes live in the gut, as it was revealed that these intestinal microbial communities have a lot of influence on the host, interest in the gut microbiota has also increased. In particular, research results showing that the intestinal microbial community is related to many diseases such as obesity, diabetes, immunity, atopy, nervous system, and cancer are being conducted in the direction of using the gut microbiota for disease treatment and diagnosis. In the treatment of intractable diseases such as ulcerative colitis, methods of changing the composition of the intestinal microbial community such as fecal microbiota transplant (FMT) have been recently tried. However, the success rate is low, and the reason for the treatment is still unknown. This is because understanding of the activities of these diseases and the intestinal microbial ecosystem is limited because the intestinal microbial community and their interrelationship, and the interrelationship between the intestinal microbial community and the host are very complex. In addition, most studies on the gut microbiota are focused on patients, and studies on the intestinal microbial community in relatively normal individuals are insufficient. It is known that the gut microbiota is affected by various factors such as heredity, diet, and lifestyle, but studies on the formation of the intestinal microbial community are still insufficient. Therefore, the purpose of this study was to improve the understanding of the structure and formation of the intestinal microbial community in healthy people by proving which factor has the most influence on the intestinal microbial community among various factors such as heredity, diet, and lifestyle. Furthermore, the difference in the intestinal microbial community between patients with ulcerative colitis treated with stool transplantation and those who have not been treated with stool transplantation is revealed, and these results are used to diagnose the success of treatment in patients before FMT. First, an experiment was conducted to find out the factors that have a more major influence on the intestinal microbial community among the genetic and environmental factors. Although genetic factors were different, naval trainees who were subject to the same environmental conditions such as exercise, diet, and sleep for a certain period were recruited as subjects. General civilian with different genetic and environmental factors were selected as control group. During the 8-week training period, the Navy trainees quit smoking, abstaining from drinking alcohol, regular sleep and training, and a balanced diet, which significantly changed environmental factors compared to before training. In the case of the control group, environmental factors did not change significantly before and after the experiment. In the case of naval trainees, there was no significant change in the alpha and beta diversity of the intestinal microbial community, but microorganisms that benefit the intestinal environment, including Bifidobacterium sp., increased, and the intestinal microorganisms causing specific diseases such as Holdemanella sp. and Turicibacter sp. decreased. In addition, among various environmental factors, the analysis was conducted by dividing into subgroups according to lifestyle in order to find the factor that has the most influence on the formation of intestinal microbial community. As a result, it was confirmed that the alpha diversity was significantly increased after the experiment in the subgroup who quit smoking during the training period, and that specific intestinal microorganisms were changed. Next, we investigated how the traditional Korean medicine “Gwakhyangjeonggisan (GJS)”, which is mainly used to treat digestive problems such as abdominal discomfort and diarrhea, has an effect on the intestinal microbial community for a long time. People who complained of abdominal discomfort at least once a month and those who did not have abdominal discomfort were recruited to take GJS twice a day for 2 weeks. People who complained of abdominal discomfort at least once a month and those who did not have abdominal discomfort were recruited to take GJS twice a day for 2 weeks. In the case of people with abdominal discomfort, the alpha diversity of the intestinal microbial community was significantly increased after ingesting GJS. Bacteroides sp., which are common intestinal microorganisms in the intestine, but are found to cause some inflammation, were significantly decreased, Subdoligranulum sp., which is known to produce butyrate, a short-chain fatty acid, to benefit the intestinal environment, significantly increased. Finally, an experiment was conducted to find out the difference in the gut microbiota between patients with and without treatment for ulcerative colitis through FMT. Stool samples were obtained several times before and after FMT treatment, and stool samples from healthy people without specific diseases were also obtained to analyze the intestinal microbial community. It was confirmed that there was a significant correlation between clinical parameters and gut microbiota. In addition, the donor's gut microbial colonization rate was not significantly related to the success of FMT. It was confirmed that there was a significant difference in the gut microbiota between the group treated with ulcerative colitis and the group without FMT. There was no significant difference between the healthy people and the intestinal microbial community in the group that did not succeed in treatment, but there was a difference in those who succeeded in treatment. The dysbiosis state was measured through various methods, and the group that did not succeed in treatment was confirmed to have no dysbiosis. Based on these results, it was found that the presence or absence of dysbiosis is related to the success of FMT. In addition, the random forest method, which is one of the machine learning methods, was used to create a model to identify patients susceptible to fecal transplantation. In this study, understanding of the composition and formation of intestinal microbial communities was improved, and in particular, the effects of various environmental factors on the intestinal microbial communities were investigated. In addition, it was confirmed that there are two types of patients with or without dysbiosis in patients with ulcerative colitis, and a model that can discriminate patients susceptible to stool transplantation through a machine learning method was created. These studies will increase understanding of the interrelationships between the intestinal microbes and the host, and it is expected that they will be useful in disease treatment and diagnosis.
