Intensive care unit (ICU) nurses provide invasive nursing practices related to indwelling urinary catheter, central venous catheter, and ventilator directly to patients, so it is very important to know and follow infection control guidelines accurately. To improve the practice of infection control by nurses, a theory-based approach is needed to understand the human behavior and the factors influencing the practice. The purpose of this study was to investigate factors influencing the infection control practice by applying the theory of planned behavior for ICU nurses in hospitals. In this study, data collection was conducted after C University IRB approval for 213 nurses who work in ICUs of 5 general hospitals with at least 350 hospital beds and located in G and C Provinces from July 1 to July 31, 2020. A total of 204 data were analyzed. The data were analyzed using the SPSS 26.0 program to conduct descriptive statistics, t-test, ANOVA, Scheffé test, Pearson’s correlation coefficient, and hierarchical regression analysis
The results were as follows. 1. The mean score for infection control practice was 3.95±0.41 out of 5 points. The mean score for the infection control practice was 3.95±0.41 out of 5 points. The score of practice related to indwelling urinary catheter was 3.86±0.40, central venous catheter was 4.05±0.33, and ventilator was 3.95±0.33.
2. For variables related to planned behavior theory, the scores for the attitude, subjective norm, and perceived behavior control regarding infection control practices of ICU nurses were 6.27±0.80, 6.53±0.58, and 5.75±0.96 points out of a maximum of 7 points.
3. There was statistically significant difference in attitude towards ICU infection control by education (F=3.66, p=.027). There were statistically significant differences in subjective norm towards ICU infection control by age (F=3.88, p=.022), and marriage (t=-2.24, p=.026). There was no statistically significant difference in both perceived behavioral control and intention for ICU infection control.
4. There were statistically significant differences in infection control practice by age (F=14.96, p<.001), total clinical experience (F=12.45, p<.001), ICU work experience (F=11.79, p<.001), marriage status (t=-3.38, p=.001), infection control training experience (t=2.63, p=.009), number of infection control trainings (F=3.74, p=.012), and infection control guideline perception (t=2.51, p=.013).
5. The infection control practice was positively correlated with attitude (r=.359, p<.001), subjective norm (r=.368, p<.001), perceived behavior control (r=.400, p<.001), and intention (r=.397, p<.001).
6. The regression model explained 34.9% of the variance in infection control practice. For general characteristics in the model, age (β=0.28, p=.012) was the only influential factor for ICU nurses’ infection control practice. In the model with added variables applying the planned behavior theory, age (β=.25, p=.011), attitude towards ICU infection control (β=.20, p=.001), perceived behavior control (β=.19, p=.010), intention (β=.15, p=.048), and subjective norm (β=.13, p=.044) were the most influential factors.
Based on the results, education and program development applying planned behavioral theory are required in order to improve the practices of infection control in the intensive care unit. In addition, age must be considered when applying education programs related to infection control.
