커뮤니티 케어는 돌봄 서비스에 대한 공공, 민간 부문 등의 주체들이 서로의 역할에 따라 돌봄 사각지대를 줄이고 서비스 접근과 통합성 향상을 통해 사회적인 유대강화를 지향하는 것이다. 보건복지부는 가정 및 지역사회가 중심이 되어 돌봄 서비스를 제공하는 지역사회 통합돌봄(Community Care)을 단계적으로 추진하기 위한 계획을 발표하였다. 이는 고령화와 저출산 등으로 인한 복지수요의 지속적인 증가와 집행 주체인 지자체의 재정악화 등으로 공공이 주관이 되어 돌봄 서비스 지원을 제공하기에는 한계가 있기 때문으로 보여 진다. 본 연구에서는 지역사회 자원들과 보건·복지서비스를 연계하여 사회 서비스를 구축·운영하고 있는 일본과 영국의 사례와 지역사회 기반의 돌봄에 있어서 오랜 역사를 지닌 독일의 커뮤니티 케어 사례, 최근 들어 커뮤니티 케어에 대한 활발한 사례연구가 진행이 되고 있는 스웨덴, 덴마크의 문헌적 고찰을 통해 분석하였다. 한국형 커뮤니티 케어의 현황을 ‘커뮤니티의 확대’와 ‘Care의 지속성’을 목표로 하는 3 Community와 3 Care의 차원에서 접근하여 한계점과 현실적인 어려움에 대해 연구 하였으며, 이를 통해 ‘포괄성’과 ‘통합성’,‘책임성’등이 바탕이 되는 이상적인 커뮤니티 케어의 추진방향에 대해서 정리하였다. 이상적인 지역사회 통합돌봄(Community Care)의 추진을 위해서는 지역사회 커뮤니티 내에 포괄적이고 다양한 서비스를 확대하고 커뮤니티 케어를 보편적으로 적용하며 노인장기요양보험을 전면적으로 확대하고 보건의료 및 요양, 복지 간의 체계적인 연계와 다양한 형태의 거주시설 확충 및 서비스 인력의 확보, 전달체계 및 지역 거버넌스의 확립, 지역의 민관협력 및 지역주민 공동체의 활성화, 사회 경제적 조직의 활성화 등이 선결되어야 할 과제라고 보이며 무엇보다도 가장 중요한 선결과제는 어떻게 재정을 확보 하느냐의 문제로 효율적인 재원의 활용과 재정방안 들에 대한 검토가 필요하다. 다시 말해 지역이 중심이 되어 이끌어 나가는 지역사회 통합돌봄(Community Care)을 추진하기 위해서는 지방분권형 재정전략이 필수적이며 정부와 로컬 거버넌스의 역할이 확대되어야 할 것이다.

Community care aims to strengthen social ties by resolving blind spots in care service through sharing responsibility among various players within public and private sectors and by improving service access and integration. The Ministry of Health and Welfare announced plans to promote community care, by definition, a home- and community-oriented care service. Such announcement is deemed to have taken place due to a limit in the community’s initiative in providing care services for reasons including increased welfare demand caused by low birth rate and the aging population and financial burden on local governments. This study analyzed cases of Japan and the U.K., nations which have built and operated social services in connection with health and welfare services, as well as community resources. The study also analyzed cases of community care within Germany, which has a long history of community-based care, and of literature reviews of Sweden and Denmark, where active case studies on community care have been conducted recently. Limitations and practical difficulties of the current status of Korean-style community care have been summarized according to the 3 Community concept, a notion which aims for “community expansion” and “care continuation”and also regarding the 3 Care concept. Under such approach, direction for an ideal community on the basis of concepts such as “comprehensiveness,” “integrity,” and “responsibility” has been outlined. To promote an ideal community care service, several societal challenges should be considered first, which includes diverse and comprehensive services within the community, general application of long-term care insurance for the aged, systematic connection among health care - community care - welfare, expansion of various forms of residential facilities, securement of service personnel, establishment of delivery systems and local governance, revitalization of local public - private partnerships and local communities, and vitalization of socioeconomic organizations. Most important challenge of all, locating financial resources is key and so the various resource utilization and financial measures ought to be reviewed. To promote community care of which the region is the principal agent, a decentralized fiscal strategy is necessary, and the role of the government and local governance will need to be expanded.
