배경: 음악을 이용한 청능 훈련은 특히 인공와우 착용자에게 효과적인 청능재활 접근법 중 하나로 많은 연구자들의 주목을 받고 있다. 그간의 연구들은 음악청능 훈련이 전반적으로 상당히 긍정적인 효과를 가져다줄 수 있는 재활 도구로 활용 가능성이 있음을 보여주었으며, 특히 인공와우 착용자가 음조, 리듬 및 음색을 얼마나 잘 인지할 수 있는지에 중점을 두어 왔다. 음악을 활용한 청능 훈련은 비교적 어려움 없이 수행할 수 있는 간단한 리듬 악기 연주 활동부터 복합적 음향 정보를 포함한 멜로디를 듣고 익히거나 노래 부르기 등 인공와우 착용자가 수행하기에 어려운 활동 등 다양한 난이도의 활동으로 구성되어 있다. 선행 연구에서는 음악 수행 능력이 전 연령에 걸쳐 발달한다고 주장하고 있지만, 대부분의 연구에서 대상자가 유소아와 성인으로 한정되어 있으며 연령에 따른 음악 수행 능력 발달에 대해 보고한 선행연구는 부재하다. 본 연구는 모든 연령대의 인공와우 착용자에게 음악을 이용한 청능재활 훈련이 미치는 영향을 파악하기 위해 메타분석을 시행하고, 그에 따른 분석 결과를 정상 청력을 가진 대상자와 비교하고자 하였다. 방법: 본 연구에서는 5개의 데이터베이스(PubMed, Scopus (Elsevier), Web of Science, ScienceDirect, and CINAHL)를 통해 2021년 1월부터 2021년 3월까지 발간된 논문을 중심으로 PICOS 기준에 따라 분석할 논문을 선정하였다. 선정된 논문의 질적 평가는 National Heart, Lung and Blood Institution (NIH)에서 제안한 NHI Tool for Assessing Risk of Bias 평가 도구를 사용하였다. 메타분석 결과의 타당성을 저해할 수 있는 출판편향(publication bias)이 있는지 확인하기 위해 깔때기 그림(Funnel plot)과 Eggers’ regression 방법을 실시하였다. 음악 훈련이 인공와우 착용자와 정상 청력을 가진 대상자에게 미치는 영향을 확인하기 위해 총 2단계에 걸쳐 메타분석을 진행하였다. 첫 번째 단계에서는 CMA (Comprehensive Meta-Analysis) 소프트웨어를 사용한 메타 분석을 통해 인공와우 착용자 그룹과 정상 청력 대상자 그룹 내에서 실험군(음악 훈련을 받은 참가자)과 대조군(받지 않은 참가자)으로 나누어 음악 훈련이 미치는 효과를 비교하고자 하였다. 두 번째 단계에서는, 첫 번째 단계에서 분석한 결과를 확장하여 Rstudio 소프트웨어를 사용한 piecewise meta regression 분석을 통해 연령에 따른 음악 훈련의 효과를 파악하고자 하였다. 결과: 본 연구에서는 두 단계의 메타분석을 수행하였으며, 44개의 문헌에서 총 893명의 대상자를 선정하였다. 전체 대상자는 청력 정도와 인공와우 착용 여부에 따라 407명의 인공와우 착용 그룹과 486명의 정상 청력 그룹으로 구분하였다. 출판 편향의 확인을 위한 funnel plot에서는 두 그룹 모두 표준화된 평균 차이 대비 표준 오차에서 균일하지 않은 결과를 보였으나, 통계적 검증을 위한 Egger's regression analysis 결과에서 출판 편향에 대한 통계적 유의미성은 인공와우 착용 그룹 (Intercept = .474, SE= 1.203, p=1.212)과 정상 청력 그룹(Intercept = 5.344, SE = 0.698, p = 0.567) 모두 검증되지 않았다. 첫번째 메타분석 결과, forest plot의 결과는 표준화된 평균 차이에서 인공와우 착용 그룹 (SMD = 0.878, p < 0.001)과 정상 청력 그룹 (SMD = 0.710, p = 0.022)에서 모두 양수로 나타났으며 이는 음악 치료를 통한 효과의 유의미성을 보였다. 그러나 그룹 별 이질성은 중간 정도로 나타났다(CI: I2=52.16, p<0.001, 정상 그룹: I2= 55.811, p<0.001). 두번째 메타분석에서 훈련을 받는 인공와우 착용자 그룹의 메타회귀분석을 수행한 결과, 0에서 21.131세의 전반적인 음악 수행력 (p =0.03, coef= 12.631; 95% CI= 10.1402 to 13.814)은 21.131에서 54.75세의 전반적인 음악 수행력(p<0.001, coef =1.714, 95% CI= 1.3210 to 3.47에 비해 더 빠르게 향상되었으며 54.57세 이상 노인의 전반적인 음악 수행력(p<0.001, coef =-1.91, 95% CI= -5.8690 to 8.3254)은 유의미하게 감소하였다. 이러한 패턴은 음악 훈련을 받지 않는 인공와우 착용자에게서도 확인되었다. 구체적으로, 0에서 21.530세 그룹의 전반적인 음악 수행력(p<0.001, coef=6.2321, 95% CI= 5.7158 to 8.9916)은 21.530에서 41.530세 그룹의 전반적인 음악 수행력 (p<0.001, coef = 1.411, 95% CI= -9.7619 to 1.5892) 비해 더 빠르게 향상되었으며 노인 그룹의 전반적인 음악 수행력효과크기는 유의한 차이(p=0.41, coef= 0.8650, 95% CI= -4.9571 to 12.7892)가 없는 것으로 나타났다. 이에 비해 음악을 훈련 받은 정상 청력의 그룹에서도 0 ~ 20.538 세의 전반적인 음악 수행력 (p<0.001; coef = 12.904, 95% CI= 10.482 to 14.327)은 가장 빠르게 향상되었으며 20.358에서 52.79 세의 전반적인 음악 수행력 (p<0.001, coef= 1.6357, 95% CI= -1.135 to -2.14)에 비해 및 52.79 세 이상 (p<0.001, coef = 6.72, 95% CI= 8.64 to 14.7)의 전반적인 음악 수행력 꾸준히 증가하였다. 이러한 패턴는 리듬 및 음조 메타 회귀 결과에서도 나타났다. 훈련을 받는인공와우 착용 그룹 메타회귀분석을 수행한 결과, 14.404세 이하의 리듬 수행력 (coef= 12.87, 95% CI 10.432 to 12.90, p<0.001)과 11.64세 이하의 음조 수행력 (p<0.001, coef = 5.25, 95% CI= 3.2741 to 6.794)은 가장 빠르게 향상되었다. 그러나 하모니와 음색의 경우 적합되지 않은 데이터로 인해 결과를 단정하기 어려웠다. 결른: 본 연구의 결과를 통해 인공와우 착용자 뿐만 아니라 건청 성인 역시 모든 연령대에서의 음악 치료의 효과를 입증하였다. 연령대에 따른 음악 훈련의 효과는 12세에서 15세 이하의 아동에게서는 음악과 관련된 영역의 향상이 가장 빠른 시기에 나타났으며, 신경가소성을 통한 독해 능력, 자기효능감, 운동 조화 능력, 논리적 사고에도 긍정적인 영향을 미치는 것으로 나타났다. 노인 역시 음악 훈련을 통한 향상을 보였으나, 그 향상의 폭은 청년이나 아동에 비해 향상의 폭이 가장 적은 것으로 나타났다. 다시 말해, 본 연구의 결과를 통해 모든 연령대에서 음악 훈련을 통한 향상을 확인할 수 있었으나, 연령대 별 향상도의 차이와 향상의 시기는 다른 것으로 나타났다. 이러한 결과는 0에서 15세의 인공와우 착용자에게 있어서 음악 훈련은 조기에 수행될수록 최대치의 향상과 이득을 얻을 수 있음을 시사한다. 또한, 인공와우 착용자의 전반적인 음악 수행력은 정상 정인에 비해서는 낮은 것으로 나타나지만 음악 훈련의 시행 여부는 인공와우 착용자 내에서도 수행력의 큰 차이를 보였다. 즉, 음악 훈련을 시행한 인공와우 사용자는 시행하지 않은 인공와우 사용자에 비해 보다 높은 전반적인 음악 수행력을 보였다. 이는 비록 인공와우 사용자의 음악 수행력은 정상 청력에 비해 낮지만, 음악 훈련을 통해 분명한 음악 수행력의 향상을 기대할 수 있겠다.

Purpose: Music training has been increasingly gaining attention of researcher with their promise able effect as one of aural rehabilitation approach especially to cochlear implant user. The vast majority of past studies have examined accuracy of pitch, rhythm, and timbre perception. Although there is some agreement that musical development occurs across the lifespan, most research conducted up until today focus on changes that occur between two specific group of childhood and adulthood and there was no past study that investigated the development of music skill with training. This study aimed to investigate the effect of music training to cochlear implant users across all age by using meta-analysis and further compare the effect to normal hearing group. Methods: Article search was done from five databases and screened by using the PICOS criteria specific for each group from five database PubMed, Scopus (Elsevier), Web of Science, ScienceDirect, and CINAHL. Selected articles’ study quality was accessed by using tools from National Heart, Lung, and Blood Institute (NIH) and risk of bias and further explored by using Egger’s regression. To confirm the publication bias, we inspect the Funnel plot and interpret that if it is distributed evenly within a triangle shape, and with Egger’s regression result indicates p value more than 0.05, no publication exists between study. The effect of music training to both normal hearing and cochlear implant user group were assessed by using two stages of analysis. In first stage, the performance of intervention group in comparison with control group to investigate the effect of music training was analyzed by using Comprehensive Meta-Analysis (CMA) software. For second stage, extending this principle, we further evaluated the relationship between music training and age were investigated by using piecewise meta regression method that have been adapted from past study to further identify the relationship and heterogeneity that exist within studies by using RStudio software. Results: Two meta-analysis were carried out in this study with total 44 articles divided into two group involving total 486 participants for the normal hearing and 407 participants for cochlear implant user. There was no evidence of publication bias from Egger’s regression test in CI group (Intercept = 6.474, SE= 1.203, p=1.212) and normal hearing group (Intercept=5.344, SE=0.698, p=0.567) even though funnel plot shown asymmetry for both groups. In the first stage of meta-analysis, result showed large effect of music training with forest plot result favor towards intervention group that receive music training for both CI group (SMD=0.878, pmeta<0.001, I2= 52.16, phet <0.001) and normal (SMD = 0.710, pmeta=0.022, I2= 55.81, phet = 00.003) group in addition to moderate statistical heterogeneity between studies. In second stage, meta-regression result for CI user revealed younger age between 0 to 21.131 years old overall music performance was rapidly increase (p <0.001, coef= 12.631; 95% CI= 10.1402 to 13.814) compared to the participants aged between 21.131 to 54.75 (p =0.03, coef =1.714, 95% CI= 1.3210 to 3.47) and for group above 54.75 years old, the overall music performance can be seen significantly reduced with (p<0.001, coef =-1.91, 95% CI= -5.8690 to 8.3254). This pattern also can be seen in CI user without music training group that shown overall music performance of participant aged 21.530 and below was the most rapid (p<0.001, coef=6.2321, 95% CI= 5.7158 to 8.9916), followed by young adult aged between 21.530 to 41.530 years old (p<0.001, coef = 1.411, 95% CI= -9.7619 to 1.5892) and no significant changes found in overall music performance in older adult aged more than 41.530 years old (p=0.41, coef= 0.8650, 95% CI= -4.9571 to 12.7892). In comparison, overall music performance of normal group that receive music training also shown that the overall music performance changes were the most rapid at age between age 0 to 20.538 years old (p<0.001; coef = 12.904, 95% CI= 10.482 to 14.327), and the overall music performance continued to increase steadily from age 20.358 to 52.79 (p<0.001, coef= 1.6357, 95% CI= 1.135 to 2.14) and 52.79 years old above (p<0.001, coef = 6.72, 95% CI= 8.64 to 14.7). This trend also was seen in the rhythm and pitch meta-regression results. Both performance of rhythm and pitch found to be mostly rapidly changes for CI users that undergoes music training aged below 14.404 years old for rhythm (coef= 12.87, 95% CI 10.432 to 12.90, p<0.001) and cochlear implant user aged below 11.62 years old for pitch (p<0.001, coef = 5.25, 95% CI= 3.2741 to 6.794). For harmony and timbre performance however, due to poorly fitted data, conclusive interpretation cannot be concluded for these two components. Conclusion: This paper provides an evidence for the benefits of active engagement with music throughout the lifespan for both individuals with hearing loss that have been fitted with cochlear implant and also normal hearing individuals. In early childhood, especially those before age 12 to 15 years old, development of music perception and skill seem to be most rapidly developed in which subsequently improve spatial reasoning, motor co-ordination, literacy and self-perceptions. Older adult group was seen to have the lowest improvement after music training compared to young adult and children. Furthermore, from this study, the effect of music training can be seen to all age group suggesting that all age group can benefits from music training but with different rate. These findings suggest that music training should be introduce earlier especially in cochlear implant user aged between 0 to 15 years old to achieve maximum benefit from music training. In addition, even though the performance of the cochlear implant users can be seen lower than normal hearing, the performance of the cochlear implant wearer with music training can be seen far better than that of the user who has not received any music training. This suggests that while performance of CI users fall short of normal hearing levels, music training can help improve the music perception of cochlear implant users.

There was no evidence of publication bias from Egger’s regression test in CI group (Intercept = 6.474, SE= 1.203, p=1.212) and normal hearing group (Intercept=5.344, SE=0.698, p=0.567) even though funnel plot shown asymmetry for both groups. In the first stage of meta-analysis, result showed large effect of music training with forest plot result favor towards intervention group that receive music training for both CI group (SMD=0.878, pmeta<0.001, I2= 52.16, phet <0.001) and normal (SMD = 0.710, pmeta=0.022, I2= 55.81, phet = 00.003) group in addition to moderate statistical heterogeneity between studies. In second stage, meta-regression result for CI user revealed younger age between 0 to 21.131 years old overall music performance was rapidly increase (p <0.001, coef= 12.631; 95% CI= 10.1402 to 13.814) compared to the participants aged between 21.131 to 54.75 (p =0.03, coef =1.714, 95% CI= 1.3210 to 3.47) and for group above 54.75 years old, the overall music performance can be seen significantly reduced with (p<0.001, coef =-1.91, 95% CI= -5.8690 to 8.3254). This pattern also can be seen in CI user without music training group that shown overall music performance of participant aged 21.530 and below was the most rapid (p<0.001, coef=6.2321, 95% CI= 5.7158 to 8.9916), followed by young adult aged between 21.530 to 41.530 years old (p<0.001, coef = 1.411, 95% CI= -9.7619 to 1.5892) and no significant changes found in overall music performance in older adult aged more than 41.530 years old (p=0.41, coef= 0.8650, 95% CI= -4.9571 to 12.7892). In comparison, overall music performance of normal group that receive music training also shown that the overall music performance changes were the most rapid at age between age 0 to 20.538 years old (p<0.001; coef = 12.904, 95% CI= 10.482 to 14.327), and the overall music performance continued to increase steadily from age 20.358 to 52.79 (p<0.001, coef= 1.6357, 95% CI= 1.135 to 2.14) and 52.79 years old above (p<0.001, coef = 6.72, 95% CI= 8.64 to 14.7). This trend also was seen in the rhythm and pitch meta-regression results. Both performance of rhythm and pitch found to be mostly rapidly changes for CI users that undergoes music training aged below 14.404 years old for rhythm (coef= 12.87, 95% CI 10.432 to 12.90, p<0.001) and cochlear implant user aged below 11.62 years old for pitch (p<0.001, coef = 5.25, 95% CI= 3.2741 to 6.794). For harmony and timbre performance however, due to poorly fitted data, conclusive interpretation cannot be concluded for these two components. Conclusion: This paper provides an evidence for the benefits of active engagement with music throughout the lifespan for both individuals with hearing loss that have been fitted with cochlear implant and also normal hearing individuals. In early childhood, especially those before age 12 to 15 years old, development of music perception and skill seem to be most rapidly developed in which subsequently improve spatial reasoning, motor co-ordination, literacy and self-perceptions. Older adult group was seen to have the lowest improvement after music training compared to young adult and children. Furthermore, from this study, the effect of music training can be seen to all age group suggesting that all age group can benefits from music training but with different rate. These findings suggest that music training should be introduce earlier especially in cochlear implant user aged between 0 to 15 years old to achieve maximum benefit from music training. In addition, even though the performance of the cochlear implant users can be seen lower than normal hearing, the performance of the cochlear implant wearer with music training can be seen far better than that of the user who has not received any music training. This suggests that while performance of CI users fall short of normal hearing levels, music training can help improve the music perception of cochlear implant users.
