연구결과: 모든 분석은 초등학교 4학년생과 중학교 1학년생으로 층화하여 분석하였다. 초등학교 4학년생의 장시간 총 스크린 기기 이용 (241분/일 이상 vs. 120분 이하)과 스마트폰 이용 (181분/일 이상 vs. 60분/일 이하)이 비만 유병률과 양의 연관성을 보였고 [총 스크린 기기 이용 - (OR: 3.33; 95% CI: 2.18, 5.09), 스마트폰 이용 - (OR: 3.57; 95% CI: 2.27, 5.61)], 장시간 컴퓨터 이용 (121분/일 이상 vs. 0분/일)과 TV 시청 (121분/일 이상 vs. 0분/일) 또한 양의 연관성을 보였지만 통계적으로 유의하지는 않았다. 중학교 1학년생의 장시간 스마트폰 이용이 비만 유병률과 양의 연관성을 보였고 (OR: 1.78; 95% CI: 1.01, 3.14), 장시간 총 스크린 기기 이용 또한 양의 연관성을 보였지만 통계적으로 유의하지는 않았다. 반면에 장시간 컴퓨터 이용과 TV 시청은 통계적으로 유의한 연관성은 보이지 않았다. 학년에 관계없이, 총 스크린 기기 이용을 수면 및 신체활동으로 대체하였을 때 비만 유병률과 음의 연관성을 보였다 [초등학교 4학년생 - 수면 시간 (OR: 0.60; 95% CI: 0.49, 0.73), 신체 활동시간 (OR: 0.77; 95% CI: 0.64, 0.93), 중학교 1학년생 - 수면 시간 (OR: 0.77; 95% CI: 0.62, 0.96), 신체 활동시간 (OR: 0.70; 95% CI: 0.55, 0.90)].

Objectives: In this study, we examined the associations of total screen time, duration of smartphone use, computer use, and TV watching with obesity prevalence, and associations between replacing screen time with other leisure-time and study-time activities with obesity prevalence in Korean children and adolescent using nationally representative survey data.
Methods: We conducted a cross-sectional analysis among 4,813 4th grade elementary students and 1st grade middle school students in the Korean Children and Youth Panel Survey 2018. Participants were asked to report weight, height, and the average duration of smartphone use, computer use, TV watching, and other leisure-time and study-time activities. We performed multivariable logistic regression to estimate odds ratios (ORs) and 95% confidence intervals (CIs) for the associations of total screen time, duration of smartphone use, computer use, and TV watching with obesity prevalence. Using isotemporal substitution model including sleep duration, physical activity, hang out with friends, conversation with parents, study, and reading, we investigated the associations of replacing total screen time or smartphone use with other leisure-time and study-time activities and obesity prevalence. We accounted for sample weight, and adjusted for potential confounders.
Results: All analyses were stratified by grade in school. In 4th grade elementary students, prolonged total screen time (≥241 compared with ≤120 min/d) and smartphone use (≥181 compared with ≤60 min/d) were associated with higher prevalence of obesity [total screen time - (OR: 3.33; 95% CI: 2.18, 5.09), smartphone use - (OR: 3.57; 95% CI: 2.27, 5.61)], prolonged computer use (≥121 compared with 0 min/d) and TV watching (≥121 compared with 0 min/d) were also associated with higher prevalence of obesity but not statistically significant. In 1st grade middle school students, prolonged smartphone use (≥181 compared with ≤60 min/d) was positively associated with obesity prevalence (OR: 1.78; 95% CI: 1.01, 3.14), and total prolonged screen time (≥241 compared with ≤120 min/d) was also positively associated but not statistically significant. In contrast, longer duration of computer use and TV watching (≥121 compared with 0 min/d) were not statistically significantly associated. Regardless of grade, replacing total screen time with sleep duration and physical activity was inversely associated with obesity prevalence [4th grade elementary students – sleep duration (OR: 0.60; 95% CI: 0.49, 0.73), physical activity (OR: 0.77; 95% CI: 0.64, 0.93), 1st grade middle school students - sleep duration (OR: 0.77; 95% CI: 0.62, 0.96), physical activity (OR: 0.70; 95% CI: 0.55, 0.90)].
Conclusions: Our data suggest that prolonged screen time, particularly smartphone use, is associated with higher prevalence of obesity, and replacing screen time with sleep duration or physical activity may be more effective in preventing obesity.
