4차 산업 혁명에 부응하여 의학 분야에서도 인공지능의 활용이 발전되어 오고 있다. 인공지능의 한 분야인 인공지능 음성 대화 시스템(Spoken Dialogue System)를 이용하여 척추 수술을 시행 받은 환자를 대상으로 통증 및 심리상태, 수면 상태 등에 대해 설문지를 기반으로 한 대화를 실시하였다. 본 연구는 척추 환자를 대상으로 인공지능 음성 대화 시스템을 활용한 최초의 연구로서 인공지능 음성 대화 시스템의 효용성 및 사용자 만족도에 대한 검증을 하였다. 대화시스템 구성 및 개발을 위해 척추수술을 위해 입원한 100명의 환자를 대상으로 수술 전과 수술 후에 인공지능 통증 설문지를 기반으로 음성 대화 시스템을 이용한 대화를 시행하여 환자의 다양한 대답과 이에 대한 요약 및 정보 분석 결과를 수집하였다. 이러한 음성 자료를 바탕으로 개발된 인공지능 음성 대화 시스템을 이용한 대화의 정확성과 의료진 및 환자 등의 사용자 만족도를 분석한 결과 7점 만족도 척도에서 환자, 간호사, 환자 모두에서 만족도가 높게 측정되었다. 정확도 및 오류에 대한 결과에서는 의사, 간호사, 환자에서 대화 인식이 되지 않아 대화를 반복한 횟수가 13, 16, 33회로 각각 측정되었다(13.5% vs 16.8% vs 34.7%). 인공지능 음성 대화 시스템이 연구대상자의 응답을 분석하였을 때 발생한 요약 내용의 오류는 의사, 간호사, 환자에서 각각 5, 0, 11회 발생하였으며(5.2% vs 0.0% vs 11.6 %), 요약 내용에서 응답의 일부가 누락된 경우는 의사, 간호사, 환자에서 각각 7, 5, 7회 발생하였다(7.3%vs 5.3% vs7.4%). 인공지능 음성 대화 시스템에 대한 본 연구 결과는 사용자 만족도 및 정확도에서 만족할만한 결과를 보였으며, 향후 의료진의 업무 부담을 줄이고 환자 상태를 효율적으로 파악할 수 있는 좋은 방법이 될 것으로 기대된다.

Objective: The purpose of our study is to develop a spoken dialogue system (SDS) for pain questionnaire in patients with spinal disease. We evaluate user satisfaction and validated the performance accuracy of the SDS in medical staff and patients. Methods: The SDS was developed to investigate pain and related psychological issues in patients with spinal diseases based on the pain questionnaire protocol. We recognized patients’ various answers, summarized important information, and documented them. User satisfaction and performance accuracy were evaluated in 30 potential users of SDS, including doctors, nurses, and patients and statistically analyzed. Results: The overall satisfaction score of 30 patients was 5.5±1.4 out of 7 points. Satisfaction scores were 5.3±0.8 for doctors, 6.0±0.6 for nurses, and 5.3±0.5 for patients. In terms of performance accuracy, the number of repetitions of the same question was 13, 16, and 33 (13.5%, 16.8%, and 34.7%) for doctors, nurses, and patients, respectively. The number of errors in the summarized comment by the SDS was 5, 0, and 11 (5.2%, 0.0%, and 11.6 %), respectively. The number of summarization omissions was 7, 5, and 7 (7.3%, 5.3%, and 7.4%), respectively. Conclusion: This is the first study in which voice-based conversational AI was developed for a spinal pain questionnaire and validated by medical staff and patients. The conversational AI showed favorable results in terms of user satisfaction and performance accuracy. Conversational AI can be useful for the diagnosis and remote monitoring of various patients as well as for pain questionnaires in the future.
