This study presents several financing methods to establish the sustainable system of National Health Insurance in Korea by cross-checking in detail of National Health Insurance systems of OECD member countries. The increase of an aged population by the growing longevity due to recent unbelievable progress in medicine and pharmacology together with remarkable developments in science technology brings about the increase of medical expenses and becomes high risk-factor in sustainability of the National Health Insurance System in Korea. Moreover, the extensions of the better insurance coverage year by year and of recent pandemic infectious disease, COVID-19, reveal the increasing trend greatly in financial expenses and it is easy to predict that the resulting accumulated deficit may pose a bigger threat to the stable service of National Health Insurance in Korea. Hence, the solid alternative revenue to cope with the growing medical expenses is needed, but the source of revenue will not be enough and that will cause bigger problems in unstability as well as in sustainability of the funding and management of the Health Service. The reason is because the present structure for a revenue streams of National Health Insurance Service in Korea still depends mainly upon insurance premiums based on earning incomes, and moreover because of recent well-known social phenomena such as the low birthrate, the low recruiting rate and the slowing economy growth. To solve these problems the rising insurance costs will not acceptable and make another problems like economic distortions, particularly in the labor market. Therefore, this is also not good solution. Hence, it is the time to discuss the methods that discover various new financial sources other than insurance premiums and establish financial resources stably in National Health Insurance Service in Korea. Based on the background mentioned above, the purpose of this study is to review the imposition of new financial resources for a sustainable health insurance system and to discuss various financing methods. To this end, the case studies of financial structure of OECD member countries that experienced health insurance financial crises ahead of Korea were examined, to apply them to National Health Insurance System in Korea in the future. As a consequence, this study presents the imposition of social security tax and various financing methods as follows. First, since the current health insurance premium system has problems like moral hazard existence of an incentive to avoid insurance premium and etc., it is a method for more stable financing by linking data from the National Tax Service, expanding the income range, and unifying the insurance premium system. Second, the imposition of the social security tax, which is relatively acceptable compared to health insurance premium, should be promoted as a policy. France introduced the social security contribution and purpose tax imposed on various incomes, while Japan promoted a consumption tax rate increase for the purpose of medical insurance and expanded financial resources stably. Accordingly, it is necessary to promote the imposition of the social security tax, which distributes the burden concentrated on earned income, and utilize the value-added tax, which has the effect of balancing between the generations, as a financial resource. Third, it is necessary to review on the imposition of a purpose tax on health risk-factors. This can be expected to have collateral effects such as cost reduction, medical expenses reduction and health promotion. Finally, it is necessary to establish a fund management system that can generate profits by utilizing health insurance reserves by bench marking successful public fund management systems. It is necessary to seek ways to expand health insurance finances by strategically managing health insurance reserves to generate additional profits.
