미국과 유럽의 선진국들은 멸균 후 의료폐기물을 유해하지 않은 의료폐기물로 취급하고 도시고형폐기물과 동일하게 취급한다. 의료폐기물은 플라스틱의 함유량이 높기 때문에 재활용 잠재력이 높다. 멸균분쇄가 가능한 유해의료폐기물은 폐연료로 재활용되거나 플라스틱 분리가 용이한 경우 물질재활용을 통해 재생원료를 생산할 수 있다. 특히, 감염성이 높은 격리의료폐기물과 유해의료폐기물은 배출기관에서 멸균분쇄를 하는 것이 2차 감염 및 오염을 방지할 수 있어 바람직하다. 따라서, 멸균분쇄시설 확대를 위한 제도개선 및 규제완화가 필수적이며 재활용을 촉진하기 위해 성상별로 멸균분쇄가 가능한 의료폐기물을 재분류할 필요가 있다. 또한, 멸균분쇄시설을 대형병원이나 중간처리업자에게도 허용하여 소규모 배출기관도 멸균분쇄처리가 가능하도록 확대해야 한다.
본 연구에서 추정한 결과, 종합병원 380개소에서 멸균분쇄시설을 도입할 시, 20년 운영기준으로 일일 222.06톤의 의료폐기물을 처리하기 위한 총 사업비는 4069.2억원이며 멸균분쇄시설을 도입했을 때와 기존 소각처리 방법의 폐기물처리비용을 비교할 시, 비용 절감효과가 약 60.6%일 것으로 추론되었다. 따라서 폐기물처리비용 절감효과를 확대하기 위해, 중소규모 배출기관에서 발생하는 의료폐기물도 멸균분쇄 할 수 있도록 법 제도가 개선되어야 한다. 또한, 멸균분쇄잔재물 재활용을 촉진하기 위한 의료폐기물 분류법을 개선하여 폐기물이 단순히 소각 후 매립되는 현 관리체계를 개선해야 한다.

This study reviewed the current Korean medical management system by dividing it into stages of discharge, stroage, transportation, and treatment, and compared it with the management system of developed countries abroad. In Korea, most of the medical waste is treated by incineration. It is transported over a long distance to be treated in a dedicated incinerator, and recyclable waste is also simply being disposed rather than being recycled. In Korea, autoclave and shredding method is promoted as an alternative treatment method to incineration, but since autoclaved residual is not allowed to be recycled, but incinerated in the final disposal stage, the purpose has been only to reduce disposal cost for healthcare institutions. Therefore, the domestic and foreign management system were reviewed to analyze the problems of current medical management system and present the way to be improve the system. Developed countries like US and European countries treat autoclaved medical waste as general municipal waste. Medical waste has a high recycling potential, because of its high content of plastic. Hazardous medical waste that can be sterilized and shredded can be recycled through waste-to-energy method or material recovery when plastic separation is rather easy. In particular, it is desirable to autoclave infectious medical waste and hazardous medical waste at the discharge, because autoclaving can prevent secondary infection or contamination due to the medical waste. Therefore, government should support expansion of autoclaving system in healthcare facilities. For expansion, medical waste should be reclassified by capability of being autoclaved. In addition, autoclaving facilities should be allowed to large-scale hospital or intermediate treatment contractor to allow small-scale healthcare facilities or institutions can treat their waste at off-site autoclave center. The treatment cost reduction effect was calculated when autoclave system is adopted to entire general hospitals in condition of year of 2020. The total project cost for treating 222.06 tons of medical waste per day was estimated to be 406.92 billion won and 60.6% of disposal fee saving is achieved.
