Background: Toll-like receptors (TLRs) that recognize pathogen-associated molecular patterns (PAMPs) are important innate immune sensors that induce anti-inflammatory and antitumor cytokines. Although a few TLR agonists have been approved for early-stage superficial cancers, treatment is limited in patients with progressive invasion cancer. Here, we identified intratumoral therapeutic role of a TLR2/3 agonist, L-pampo, which promotes antitumor immunity and enhances the immune checkpoint blockade.
Method: L-pampo was manufactured by combining a TLR2 agonist, Pam3CSK4 with a TLR3 agonist, Poly(I:C). Cytokines were measured after L-pampo treatment with the co-culture of spleen cells and macrophages. MC38, B16F10 Tumor-bearing mice were intratumoral treated four times with 50μL of L-pampo every 3 days. With the same schedule, Immune checkpoint inhibitor (anti-PD-1, CTLA-4) was Intraperitoneal injected, and their tumors were measured. The tumors, lymphoid and spleen were analyzed using flow cytometry, histological, and Nanostring methods.
Results: L-pampo treatment induced robust activation of Th1 and Th2-mediated immunity and immunogenic cell death. Intratummoral L-pampo treatment increased the number of innate immune-related activity and cells in the tumor. Moreover, tumor suppression was demonstrated through the mediated immune response of CD8+ T cells and IFN- γ. L-pampo simultaneously activated TLR2 and TLR3 signaling, thereby extensively changing the immune-related gene signatures within the tumor microenvironment. Tumor-specific CD8+ T cells are increased in the spleen and lymph nodes, leading to abscopal effects that inhibit untreated distant tumors. In particular, L-pampo treatment combined with PD-1 and CTLA-4 further improved the efficacy of monotherapy, leading to complete tumor regression and overall long-term survival. Furthermore, the combination immunotherapy based on L-pampo induced continuous antitumor immunity through memory immunity in colon cancer and melanoma.
