Purpose: Osteoporosis is a disease that affects bones of the whole body. Previous studies of artificial intelligence (AI) for predicting osteoporosis on panoramic radiographs have used cropped images that include mandibular inferior cortical bone. This study aimed to investigate sensitive area for AI to predict osteoporosis on full-size panoramic radiographs. Materials and Methods: Panoramic radiographs were selected from 1,156 female patients (mean age 49.0±24.0 yrs). All panoramic radiographs were diagnosed as osteoporosis or normal by oral and maxillofacial radiologists. Among 537 radiographs of osteoporosis patients (mean age 73.7±7.8 yrs), 383 (mean age 74.0±7.6 yrs) were used for training data and 72 (mean age 73.7±8.0 yrs) for testing. Among 619 radiographs of normal patients (mean age 27.5±5.4 yrs), 433 (mean age 27.7±5.4 yrs) were used for training data and 93 (mean age 26.4±5.1 yrs) for testing. VGG16 deep learning convolutional neural network model was used for prediction of osteoporosis and normal. Gradient-weighted class activation mapping (Grad-CAM) visualization was performed for determining the sensitive area for VGG16 to diagnose osteoporosis. Results: The accuracy of predicting osteoporosis cases was 100%. In 72 osteoporosis patients predicted as having osteoporosis by AI, the sensitive area was at cervical vertebrae in 70.8% of the cases (51/72) and 72.2% (52/72) for mandibular inferior cortical bone, 30.6% (22/72) for cranial base, 33.3% (24/72) for mandibular cancellous bone, 20.8% (15/72) for maxillary cancellous bone, 8.3% (6/72) for zygomatic bone, and 5.6% (4/72) for teeth area. Conclusions: The cervical vertebrae and mandibular inferior cortical bone were sensitive area in predicting of AI for osteoporosis on full-size panoramic radiographs.
