국가의 장애인정책종합계획에 의거하여 서비스지원 종합조사, 개인예산제 시범서비스 등 개인별 욕구, 환경 등을 고려하는 장애인의 자립을 위한 맞춤형 통합지원 복지 정책이 시행되고 있다. 또한, 우리나라는 2025년 초고령사회로 진입을 앞두고 있고, 이에 따른 장애 범주의 확대로 지원 복지서비스의 일환인 보조기기의 수요 증가와 이에 대응하는 공적급여 예산이 2019년 3,601억원에서 2021년 4,077억원으로 증가되는 등 앞으로 국가재정 부담이 점차 증가할 것이다. 이러한 시점에 보조기기 지원 서비스의 효과를 확인하고 공적급여 예산 효율성을 담보할 수 있도록 보조기기가 장애인의 자립 등에 어떤 중재효과가 있는지에 대한 여러 근거 확보가 필요하다. 이에 본 연구에서는 장애인 유형 중 가장 사례가 많은 지체장애인 대상으로 보조기기가 자립에 어떤 영향을 주는 지를 알아보기 위해 체계적 문헌고찰을 진행하였고 자립생활을 위한 장애인 측면에서의 기능수행능력과 삶의 질에 대한 영향을 분석하였다. 2001년부터 2022년까지 국내‧외 문헌 조사 및 분석을 PRISMA(Preferred Reporting Items for Systematic Reviews and Meta-Analyses) 흐름도(Moher et al., 2009)에 따라 진행하여 171편이 검색되었고 총 14건에 대해 분석하였다. 본 연구결과와 시사점을 3가지로 요약하였다. 첫 번째로, 지체장애인의 보조기기 중재로 인한 기능수행능력 변화에 대해 총 9편의 문헌에서 이동능력과 에너지소모량 등 이동능력의 효율성 측면에 대해 개선되는 점을 확인할 수 있었다. 또한 상지 기능 중심의 일상수행능력에 대해 휠체어 랩보드 사용을 통한 컴퓨터 사용 활동 개선 등 중재효과가 있음을 알 수 있었고 보조기기 사용으로 주변 환경에 대한 도전(어려움) 정도가 낮아짐을 알 수 있었다. 특히, 클라이언트의 개선요구 등이 담긴 보조기기 개발과정 내지 클라이언트의 욕구 및 배경요인 등의 상담 과정 등을 거친 보조기기 중재가 이루어져 사용자의 욕구나 기능수준을 고려한 중재 과정이 있음을 대부분의 문헌에서 확인 할 수 있었다. 이는 장애인에 욕구와 기능수준에 맞는 맞춤 지원을 위해서는, 사회복지사 등이 참여하는 서비스지원 종합조사에서 이러한 개인의 욕구나 환경에 대해 체계적인 파악이 필요하고, 서비스지원 종합조사결과와 보조기기 서비스 중재결과를 데이터베이스화하여 이후의 서비스지원 종합조사와 보조기기 중재에 환류가 되어질 필요가 있음을 보여주고 있다. 그리고 보조기기 연구개발사업 등에도 장애인 당사자 등 클라이언트의 다양한 요구가 전달 되어져서 수요기반 보조기기가 지속적으로 개발 될 수 있도록 사회복지 실천현장에 반영, 제도화할 필요가 있음을 의미한다. 또한 보조기기 중재시 적합한 보조기기가 중재될 수 있도록 서비스에 활용될 수 있는 보조기기 품목별, 제품별로 보조기기 중재 대상, 효과에 대한 근거 문헌이 확보되어야 하며 이에 대한 근거 확보를 위한 보조공학사, 사회복지사, 의료인이 협력하는 연구가 지속되고 이를 복지서비스단계인 사회복지 실천현장에 전달하여 활용될 수 있도록 할 필요가 있겠다. 본 논문도 근거 확보를 위한 일환이다. 두 번째로, 6편 문헌에서 보조기기 중재로 삶의 질이 전반적으로 개선됨을 확인하였다. 특히, 중재된 보조기기의 개수보다 보조기기에 대한 사용만족도가 삶의 질 척도에 유의미한 양의 상관관계가 있음을 보고하였다. 다만, 한 문헌에서는 척수손상 장애인 5명에 대해 기계적 외골격보행보조기에 대한 중재에 대해 전반적인 삶의 질은 개선되었으나 신체적 건강측면의 삶의 질에 대해서는 만성 완전손상 척수장애인의 특성상 신경학적 운동기능회복이 어려워 삶의 질 향상이 유의하게 이루어지지는 않았다고 하였다. 하지만, 저하보다는 유지되는 측면에서도 보조기기 중재의 의미가 있음을 해당 문헌의 저자는 제시하고 있었다. 이는 만성기에 들어선 장애인의 경우 기능수행능력, 삶의질 등이 저하되지 않고 유지되는 측면에서도 보조기기 중재 의미가 있음을 제시하고 있어 향후 보조기기 중재 효과분석에 있어 기능수행능력 등의 향상 뿐 아니라 유지되는 정도 또한 주요하게 다룰 필요가 있겠다. 세 번째로는 지체장애인의 기능능력수행 또는 삶의 질에 긍정적 영향을 준 보조기기에 대해서는 총 14편의 문헌에서 이동 보조기기가 13건(41%), ADL 보조기기가 3건(10%), 경사로 등 환경제어 보조기기가 3건(10%), 자세유지 보조기기가 2건(6%), 욕창방지 보조기기가 2건(6%), 재활치료 보조기기 2건(6%), 작업활동용 보조기기가 2건(6%), 이승 보조기기가 1건(3%), 차량용 보조기기 1건(3%), 의사소통용 보조기기가 1건(3%), 위생용 보조기기 1건(3%), 의지보조기 1건(3%) 순으로 총 32건의 보조기기가 확인되었다. 이는 2020년 장애인실태조사의 결과인 이동관련 보조기기의 수요도와 보유도가 타 보조기기보다 상대적으로 높은 것과 일치하였다. 이동관련 보조기기를 지체장애인이 필요로 하고 이를 보유하고 있는 것으로 보아 지체장애인의 이동을 통한 다양한 사회활동 욕구가 그만큼 우선시 되어야 하고 이에 대응하는 이동 관련 보조기기가 폭넓게 개발되고 지원될 필요가 있겠다. 앞으로 보조기기는 장애인 등의 일상생활 및 사회생활을 지원하는 기술로 고령화에 따른 장애인구 증가 등 다가오는 초고령사회의 복지정책 수립에 한 축을 담당할 것이다. 다양한 장애유형에 대한 맞춤형 지원정책에 보조기기를 통한 지원을 포함하기 위해 보조기기 중재 효과 등 보조기기 지원근거 확보가 필요하며 이를 위한 기초자료로서 본 연구가 가지는 의미가 있다. 그리고 본 연구와 같이 보조기기 중재효과로 기능수행능력 등 보조기기의 임상적 효과 외에도 삶의질, 사회적 효과, 경제성 등에 대해서 사회복지 관점의 연구 등이 지속될 필요가 있으며 이들 결과를 관련 보건복지 서비스, 정책 등에 적용할 필요가 있다.

The results and implications of this study were summarized in three points. First, a total of 9 literatures on changes in functional performance ability due to assistive device intervention for the people with physically disabilities showed improvement in the mobility performance and the mobility efficiency aspects such as an energy consumption. In particular, it was confirmed in most of the literature that there was a process in which the client's needs and functional level were considered through the assistive device development process containing the client's request for improvement, or the assistive device intervention through the counseling process such as the client's desire and environment factors. Therefore, it is necessary to grasp the individual's needs or environment in the comprehensive service support survey, and the results of the comprehensive service support survey and the assistive device service intervention result are made into a database to be fed back to the subsequent comprehensive service support survey and assistive device intervention. It should be institutionalized that the various demands of the people with disabilities be provided to the assistive device R&D project so that necessary assistive devices can be continuously developed.
