Modern people are experiencing physical and mental difficulties such as stress and lifestyle-related diseases while living in an era of excessive competition and rapid change. Mental illness is so common that it is said that every modern person has at least one mental illness. According to the Ministry of Health and Welfare's “Survey on Mental Illness (2021),” the lifetime prevalence of mental illness is 25.4%, and it can be estimated that one out of four adults experience mental health problems at least once in their lifetime. It is known that most mental disorders can have a good prognosis through early detection and early intervention, and daily life can be maintained through continuous treatment. Since the 1960s, the mental health system of developed countries, including the United States, has shifted the paradigm toward deinstitutionalization and community mental health. In accordance with this trend, Korea also revised the Mental Health Act in 1997 and implemented a policy to improve the human rights of the mentally ill. The improved policy avoids long-term hospitalization for people with mental illness and supports various welfare services to help people with mental illness settle down and live in the community. The Mental Health Welfare Center plays a pivotal role in helping mentally ill people settle into the community through counseling and case management services. In the field of case management for mentally ill patients, psychiatric, psychological, and philosophical counseling approaches are needed for their recovery. Philosophical counseling can give answers to questions such as ‘Who am I?’ and ‘How should I live?’ to those living with mental problems. Existential philosophy, in particular, does not stop at a specific attitude toward human beings, but can be a methodology needed to understand the unique nature and problems of human beings and to find solutions suitable for them. This paper examines the existence of the mentally ill through Kierkegaard’s philosophy of despair and self-being, and suggests a direction for ‘how they should live’. Kierkegaard’s philosophy urges his clients to make an existential decision, to accept themselves, to take on their existence in the here and now. This paper consists of four main parts. The first part analyzes Kierkegaard’s despair and three stages of existence. This concept shows how human existential despair can be resolved. Here, we can understand that human beings are beings that create themselves. The second part presents the cases of clients who are receiving case management services at the Mental Health Welfare Center, and shows the need for philosophical counseling in their recovery journey. In the third part, the cases is analyzed based on Kierkegaard's despair and the three-stage model of existence, which is a self-dialectic to resolve despair, and the three-stage model of existence is restructured and applied to the cases. Through this, it is suggested that clients with psychiatric diagnoses live as their own existence and that the existential leap through establishing relationships is applied as an indicator of their recovery. Lastly, in the process of their existential leap, the establishment of a relationship with a counselor, a case manager, is a stepping stone for an existential leap, and it is examined that philosophical counseling in the field of case management is philosophical practice.
