제1장 미세먼지 노출과 인지기능 장애(치매, ADHD)와의 연관성에 대한 체계적 문헌고찰 PM10 노출이 치매 및 ADHD에 끼치는 영향을 살펴보고자 체계적 문헌고찰을 통해 메타분석을 수행하였다. 두 개의 검색원인 Ovid-MEDLINE, Ovid-Embase에서 ‘치매’, ‘주의력결핍 과잉행동장애’, ‘PM10’ 검색어를 활용하여 검색하였다. 총 1,045편의 문헌이 검색되었으며 사전에 정의한 선택/배제 기준을 토대로 최종 25편의 문헌을 선택하였다. 치매와 관련된 문헌 14편 중 10편의 문헌을 대상으로 메타분석을 수행한 결과, 통합추정치는 1.21 (95% CI 1.02-1.43)로 유의한 연관성이 확인되었다. 주의력결핍 과잉행동장애와 관련된 문헌 11편 중 7편을 메타분석한 결과, 통합추정치는 1.52 (95% CI 0.86-2.67)로 PM10 노출에 따른 ADHD 질병과의 연관성은 증가하였으나, 통계적으로 유의하지 않았으며 문헌들 사이의 이질성이 높아 미세먼지 노출의 영향은 결론내리기 어렵다고 보았다.
제2장 고령인구에서 미세먼지 장기 노출이 치매 발생 위험에 끼치는 영향 본 연구는 2008년부터 2019년까지 PM10 장기 노출이 치매 발병 위험에 끼치는 영향을 분석하였다. 국민건강보험공단 맞춤형 DB를 활용하여 2008-2009년 사이 1회 이상 국민건강검진 프로그램에 참여한 65세 이상 고령인구를 대상으로 전국적인 후향적 코호트 연구를 설계하였고, 코호트 시작일(2008년 1월 1일)부터 치매 발생일, 사망일, 거주지 이동일 또는 연구 종료일(2019년 12월 31) 중 가장 먼저 발생한 일자를 기준으로 환자를 추적관찰하였다. Aims The aim of this thesis is to investigate the association between long-term particulate matter (PM10) exposure and cognitive dysfunction including dementia and ADHD in Korea. The thesis consists of three parts. Part 1 reviewed systematically to examine the association of particulate matter on cognitive health. Part 2 focuses on determining the relationship between long-term exposure to PM10 and the risk of developing Alzheimer's disease and vascular dementia in the elderly using data from the National Health Insurance Corporation (NHIC). A systematic review of the association between exposure to PM10 and cognitive dysfunction (dementia and ADHD) This systematic review of epidemiological studies aimed to investigate the relationship between the exposure of PM10 and dementia and ADHD and identify gaps in our current knowledge. We searched the Ovid-MEDLINE and Ovid-Embase databases using the search terms ‘dementia’, ‘attention deficit hyperactivity disorder’, and ‘PM10’. The keyword search yielded 1,045 results. Twenty-five studies met our inclusion criteria. As a result of meta-analysis of 10 out of 14 literatures related to dementia, the pooled estimate was 1.21 (95% CI 1.02-1.43). As a result of meta-analysis of 7 out of 11 literatures related to ADHD, the pooled estimate was 1.52 (95% CI 0.86-2.67). Although there was an association between PM10 exposure and ADHD, it was considered difficult to conclude the effect of particulate matter exposure on cognitive impairment due to high heterogeneity and inconsistent statistical significance.
Part 2. Long-term particulate matter exposure and the risk of Alzheimer’s disease and vascular dementia The study populations included those aged 65 years and above who participated in at least one national health checkup program from 2008 to 2009 in NHIC data. A nationwide retrospective cohort study was designed, and patients were followed from the date of cohort entry (January 1, 2008) to the date of dementia occurrence, death, moving residence, or the end of the study period (December 31, 2019), whichever came first. Extended Cox proportional hazard models with time-varying exposure were used to estimate hazard ratios (HR) for Alzheimer’s disease and vascular dementia. A total of 1,436,361 participants were selected, of whom 167,988 were newly diagnosed with dementia (134,811 with Alzheimer’s disease and 12,215 with vascular dementia). The results show that for every 10µg/m3 increase in PM10, the HR was 0.99 (95% CI 0.98-1.00) for Alzheimer’s disease and 1.05 (95% CI 1.02-1.08) for vascular dementia. Stratified analysis according to sex and age group showed that the risk of vascular dementia was higher in men and in those under 75 years of age. In the sensitivity analyses, the robustness of the results was confirmed by outcome definitions, exposure time windows, deaths, and regional indicators.
Part3. Association between PM10 and ADHD in Children and Adolescents Data were obtained from the Korean National Health and Nutrition Examination Survey 2008–2018. Outcomes were defined from parental reports of ever doctor-diagnosed ADHD. And ADHD cases were matched to non-cases with 1:10 age-sex matching. Individual exposure levels were assigned according to each study participant’s administrative address during the year of diagnosis. Multivariate logistic regression models were used to estimate odds ratios (ORs) and 95% confidence intervals (CIs). After age–sex matching at a 1:10 ratio, the final study participants comprised 1,120 children aged 6–19 years. A unit increase in the PM10 concentration was significantly associated with ADHD (OR, 1.44; 95% CI, 1.02–2.02 per 10µg/m3). The association with ADHD was stronger at higher quartiles than in lower quartiles of PM10 exposure; however, it was not statistically significant. Our results suggested that long-term PM10 exposure was associated with increased ADHD in children and adolescents.
Conclusion: Based on a systematic literature review of existing epidemiological studies, the findings suggest a potential association between PM10 and dementia and ADHD. However, the evidence regarding this relationship remains unclear. These findings add to the evidence for an association between particulate matter and dementia, it may be more related in terms of blood vessel damage. Additionally, it was suggested that environmental risk factors, including air pollution, have an impact on children and adolescents.
