Radical prostatectomy is one of the most effective methods to cure prostate cancer and is performed in 65% of prostate cancer patients. However, due to the complex anatomical structure and fragile surrounding tissues, this surgery often leads to common damage to surrounding muscles, and nerves. As a result, patients who undergo radical prostatectomy continue to experience issues such as urinary dysfunction, sexual dysfunction, and pain even after the completion of treatment. These issues are not only associated with problems such as depression and fatigue but ultimately contribute to a decrease in the quality of life. The lower urinary tract symptoms commonly experienced by patients who have undergone radical prostatectomy are one of the major factors influencing the quality of life of prostate cancer patients. Although the importance of these symptoms is consistently evident, there is scarce research investigating the differences in the degree of lower urinary tract symptoms and their impact on life quality according to the postoperative period. Therefore, this study aims to compare the differences in the quality of life of prostate cancer patients who have undergone radical prostatectomy according to the postoperative period and to examine disease-specific aspects of lower urinary tract symptoms, physical aspects of fatigue, and psychological aspects of depression. This research can serve as foundational data for the development of tailored nursing interventions reflecting the characteristics of prostate cancer patients during the postoperative period.
The collected data were analyzed using the IBM SPSS/WIN 25.0 statistical program, employing methods such as frequency and percentage, mean and standard deviation, minimum and maximum values, chi-square test, independent t-test, ANOVA, Scheffé test and Pearson’s correlation coefficient.
The results of this study are as follows.
1. The mean for participants’ lower urinary tract symptoms was 13.34±6.28 (instrument range: 0-35), and mean score was 1.91±0.89 (scale range: 0-5). The mean for Depression was 7.15±4.04 (instrument range: 0-21) and mean score was 1.02±0.57 (scale range: 0-3). The mean for Fatigue was 10.17±6.92 (instrument range: 0-52) and mean score was 0.78±0.53 (scale range: 0-4). The mean for the quality of life was 71.57±13.92 (instrument range: 0-108) and mean score was with the subdomain indicating the quality of life in the physical domain being the highest at an average of 24.00±3.38 points. The mean score of each subcategory was 24.00±3.38 for the physical well-being, 13.10±6.24 for social/family well-being, 17.95±3.78 for emotional well-being, and 2.36±0.92 for functional well-being.
2. The lower urinary tract symptoms showed statistically significant differences based on the postoperative follow-up period. Specifically, participant with less than 12 months of postoperative follow-up exhibited significantly higher symptoms compared to those with 12~24 months and those with 24 months or more (F=5.997, p=.003). Depression did not show statistically significant differences across the follow-up periods. Fatigue demonstrated statistically significant differences based on the postoperative follow-up period. Participant with less than 12 months of follow-up had significantly higher fatigue compared to those with 12~24 months (F=5.610, p=.005). Quality of life did not exhibit statistically significant differences across the follow-up periods.
3. In participant with less than 12 months of postoperative follow-up, the quality of life showed a significant negative correlation with depression (r=-.616, p<.001) and fatigue (r=-.557, p<.001). In participant with 12~24 months of postoperative follow-up, the quality of life showed a significant negative correlation with depression (r=-.399, p=.007) and fatigue (r=-.315, p=.037). In participant with 24 months or more of postoperative follow-up, the quality of life showed a significant negative correlation with urinary tract symptoms (r=-.569, p<.001) and fatigue (r=-.477, p<.001).
In summary, the quality of life of prostate cancer patients after radical prostatectomy was significantly correlated with depression and fatigue in patients with less than 24 months of postoperative follow-up, and with lower urinary tract symptoms and fatigue in patients with more than 24 months postoperatively. Therefore, it is necessary to consider the differences in influencing factors according to the postoperative period, and to ensure that the significance of lower urinary tract symptoms in not overlooked in patients with postoperative period of 24 months or more when the development of nursing interventions to improve the quality of life of prostate cancer patients with radical prostatectomy.
