mRNA vaccine emerged as promising candidate worldwide for the COVID-19 pandemic due to their relatively rapid development and production. However, in comparison to conventional vaccines such as protein-based vaccines, there is limited research on their adverse effects and mechanisms. In this study, safety profile of mRNA vaccines (COVID-19) is evaluated by comparing hematological variations with conventionally used protein conjugate vaccines (Meningococcal vaccine). The study aimed to identify important biomarkers associated with mRNA vaccine- induced adverse events. The results showed that participants who received the mRNA vaccine experienced more injection site pain and systemic adverse events than those who received the conventional vaccine. Furthermore, Hematological, and serological analyses revealed a decrease in white blood cell (WBC) count and an increase in C-reactive protein (CRP) and haptoglobin levels after mRNA vaccination. Proteomic analysis revealed that the mRNA vaccine group had a significant increase in nine specific proteins compared to the conventional vaccine group. Most of these proteins belonged to the complement component or acute phase proteins. These findings suggest that adverse events following mRNA vaccination are related to an inflammatory response mediated by complement components and acute phase proteins. In conclusion, this study is significant as a foundational study that provides insight of side effects, which is essential for mRNA vaccine development.
