Analysis of expedited programs for gene therapy products: comparison in the United States, the European Union, Japan, and South Korea
Gene, Cell, & ...

Background: Gene therapy products (GTPs) are being developed in an effort to address unmet medical needs for incurable diseases and find new, better treatments. GTPs can be used for diseases that are difficult to treat with current treatment methods, such as malignant tumors and genetic diseases, and development is continuously expanding despite controversy over risk and safety. Objective: Regulatory agencies are implementing a variety of expedited programs to promote early development and commercialization of GTPs. Therefore, we sought to analyze and compare the expedited programs being implemented in the US, EU, Japan, and South Korea and the GTPs approved through the expedited programs. Methods: In this cross-sectional study, we searched regulatory agency websites in the US, the EU, Japan, and South Korea until August 25, 2023 to collect information on expedited programs available for GTP. Additional information was explored in available related documents or in the Frequently Asked Questions section. Additionally, approved GTPs were reviewed on the Gene, Cell, & RNA Therapy Landscape Report, Q2 2023 published by American Society of Gene & Cell Therapy and searched on regulatory agency websites in the US, the EU, Japan, and South Korea to determine whether they received an expedited program. Results: Applicants manufacturing GTP can receive expedited programs that are applicable to both general drug and regenerative medicines for accelerating drug development, marketing application review, and marketing approval. Except in the US, orphan drug designation also supports expedited processing. Each of the expedited programs implemented by the regulatory agencies studied had similar characteristics and benefits, but there were some differences in the conditions required for specific requests. A total of 21 GTPs were approved, 16 in the US, 14 in the EU, 8 in Japan, and 3 in South Korea. Most of the included GTPs underwent one or more expedited programs for early patient access. Conclusion: Although expedited programs share similar functions, they have different legal bases, making comparison and harmonization difficult. Therefore, scientific discussions between regulatory agencies are encouraged to harmonize the requirements of the expedited program.
