목적 본 연구에서는 임상/역학 연구 등의 관찰 연구에서 대사증후군과 대사증후군의 구성요소들 (비만, 당뇨병, 고혈압, 이상지질증 등의 대사성질환 및 이와 관련된 양적 형질) 사이에 관측되는 질병 발생의 강한 상관성이 유전적인 요인의 상관성에 기반한 것인 지를 확인하고자 하며, 더 나아가 대사증후군의 위험요인으로서 대사증후군 구성요인들이 유전적 요인에 기반한 대사증후군과의 유전적 인과관계에 기인하여 질환 발생에 영향을 미치는 지를 규명하고자 한다.
방법 국립보건연구원의 한국유전체역학코호트 사업을 통하여 생산된 약 72,000명에 대한 역학 정보와 한국인 칩 분석을 통해 확보된 유전체 정보를 연계하여 대사증후군 및 구성요소들에 대한 유전 요인 발굴을 위한 전장유전체연관 분석을 시도하였다. 전장유전체연관분석의 결과로부터 대사증후군과 구성요소들(비만, 당뇨병, 고혈압, 이상지질증 등의 대사성질환 및 이와 관련된 양적 형질) 사이의 유전적 상관관계는 LDSC 소프트웨어를 이용하여 분석하였다. 대사증후군과 구성요소들 사이의 인과관계 분석은 멘델리언 무작위 분석 (Mendelian Randomization, MR) 방법 중 inverse-variance weighted (IVW) 방법을 이용하여 수행하였다.
결과 대사증후군과 대사성 질환의 유전적 상관관계 분석 결과 대사증후군은 이상지질혈증(rg=0.95)과 제2형 당뇨병(rg=0.42)과 각각 유의한 상관관계를 보였다. 대사증후군과 구성요소들과의 인과성 분석을 위한 멘델리언 무작위 분석 결과는 당뇨병, 이상지질혈증, 비만, 고혈압과 같은 질환들은 대사증후군에 대하여 양의 인과관계를 보였다. 대사증후군 관련 양적 형질들의 경우, 공복혈당, 당화혈색소, 중성지방, 총콜레스테롤, 수축기혈압, 허리둘레는 대사증후군에 대하여 양의 인과관계를 보인데 반하여 고밀도 지단백 콜레스테롤과 이완기 혈압 수치는 대사증후군에 대하여 음의 인과관계를 보였다.
결론 본 연구를 통하여 다수의 대사증후군 구성요소들은 유전적으로 대사증후군과 연관이 있으며, 이들이 대사증후군의 발생에 영향을 미치는 대사증후군과의 유전적 인과성도 확인하였다. 이로부터 대사증후군 발생에 관한 유전적 배경의 이해와 질환 기전에 대한 분자생물학적 이해를 제공하였다.

Results Genetic correlation analyses demonstrated that MetS shows a positive correlation with dyslipidemia (rg value = 0.95, P = 2.27 X 10-18) and type 2 diabetes (T2D) (rg value = 0.42, P = 8.98 X 10-5). A causal relationship was detected between MetS and component diseases (T2D, dyslipidemia, hypertension, and obesity) as well as several related quantitative traits (fasting plasma glucose, hemoglobin A1c, triglyceride, high-density lipoprotein cholesterol, weight, waist-hip ratio, and waist circumference) by MR analyses.
Conclusion Our study implies a shared genetic basis underlying MetS and related traits and provides novel insights into the biological mechanisms underlying these complex traits. Our study further informs public health intervention by supporting the important role of metabolic quantitative trait management in preventing MetS and related diseases.
