목적 결핵성과 화농성 척추염에서 추간판의 조영증강 자기공명영상의 형태를 분석하고 두 질환간의 감별에 도움이 되는 지를 알아보고자 하였다. 대상과 방법: 감염성 척추염을 가진 31명의 환자에서 감염된 척추체 사이에 존재하는 43개 추간판의 자기공명영상 소견을 후향적으로 분석하였다. 척추염의 원인으로 결핵성이 19명, 화농성이 12명이었다. 자기공명영상의 조영증강 T1강조 축상면과 시상면을 이용하여 추간판의 조영증강 형태를 분석하였다. 추간판의 조영층강 형태는 I형; 조명증강이 없는 경우. II형; 추간판의 변연부의 조영증강이 있는 경우, 111형 추간판의 변연부와 척추종판과 인접한 추간판 중앙부위의 조영증강이 있는 경우, IV형. 추간판에 전체적인 조영증강이 있거나 완전 파괴가 있는 경우로 하였다 결과: 19명의 결핵성 척추염에서 28개의 추간판을 침범하였다 조영증강 형태는 I형 (n=4,14%), II형 (n=17 61%). 111형 (n=1. 4%) IV형 (n=6, 21%) 이었다. 12명의 화농성 척추염에서는 15개의 추간판을 침범하였다 조영증강 형태는 I형 (n=1 7%), II형 (n=2.13%), 111형 (n=10, 67%) IV형 (n=2. 13%)으로 나타났다. 결론: 감염성 척추염의 자기공명영상에서 추간판 조영증강 형태의 주의깊은 분석은 결핵성과 화농성 척추염을 구분하는데 있어 유용한 감별점이 될 것으로 사료된다.

Materials and Methods: Magnetic resonance images obtaIned in 31 consecutive infectious spondylitis patients (43 discs) in whom intervention occurred due to infected vertebral bodies, were retrospectively analysed. After analysis, the Patterns of disc enhancement revealed by contrast-enhanced T1-weighted axial and sagittal MR imaging were classified as of four types: Type I, non-enhancing; Type II, enhancement of the peripheral margin of the disc; Type III, enhancement of the peripheral margin and central area adjacent to the cortical vertebral endplate; Type IV, general enhancement and/or destruction of the disc. Results: There were 19 cases of tuberculous spondylitis involving 28 intervertebral discs, and the enhancement patterns observed were as follows: Type I: n=4, 14%, Type II. n = 17,61%, Type III: n= 1, 4%, Type IV: n = 6, 21%.
