The study was based on a series of 135 patients treated as emergency patients at Wonju Christian Hospital, from Jan. 1, 1997, to Dec. 31, 2001. The postoperative bleeding was the most frequent cause of active oral bleeding in emergency room and bleeding from trauma and medically compromised (bleeding disorders) patients were next in order of frequency. In the injury of maxillofacial vessels, peak incidence was occurred in the inferior alveolar vessel (42.9%), followed by the submucosal vessel of lip & cheek, the superior alveolar vessel, and sublingual vessels. The most common disease of bleeding disorders was vascular wall alteration (infection, etc), followed by liver disease, thrombocytopenic purpura, anti-coagulation drugs in order. In the characteristics of dental diseases on bleeding disorders, periodontal disease and alveolar osteitis (osteomyelitis) were more common. The hemostasis was most obtained by use of wound suture, simple pressure dressing, drainage for infection control and primary interdental wiring of fracture. In the complication group, the infrequent incidence was showed in vomiting, hypovolemic shock, syncope, recurred bleeding & aspiration pneumonia. In the uncontrolled oral bleeding, the injured vessels were suspected as skull base & ethmoidal vessels. In this study, authors found that the close cooperation between the dentistry (Oral and maxillofacial surgery) and the medicine (emergency & internal medicine) was the most important for early proper control of active oral bleeding. And then post-operative wound closure, drainage for infection control and previous systemic evaluation of bleeding disorders were critical for the prevention of postoperative bleeding in the local dental clinic.

대상 데이터
연구대상은 1997년 1월 1일부터 2001년 12월 31일까지 원주기독병원 응급실에 구강내 과다출혈을 주소로 내원한 135명의 환자를 대상으로 하였다.
성능/효과
1. 가장 흔한 출혈의 원인은 수술후 출혈(43.5%)이었고 손상(사고)으로 인한 출혈, 내과질환에 의한 출혈,출혈성 병소 순이었다.
4. 출혈성 질환별 분류에서 구강내 과다출혈의 원인으로는 창상 감염에 의한 혈관벽의 변화가 54.9%로 가장 높았고, 간(liver)질환,백혈병 같은 혈소판 감소성 자반증, coumarin 등의 항응고제 투여자 순이었다.
5. 출혈성 질환자에서의 치과질환 특성은 치주질환(39.5%)이 가장 많았고 치조골(수)염,점막질환 순이었다.

구강내 출혈의 원인별 분포에서는 발치 등의 수술후 출혈이 64예(43.5%)로 가장 많았고 손상으로 인한 과도한 출혈 43예(29.3%), 내과적인 출혈성 질환에 의한 출혈 37예(25.2%), 혈관종 등 출혈성 구강악안면 병소에 의한 출혈이 소수 있었다 (Table 3).
전체 135명의 출혈 환자들 가운데 합병증 발생 내용을 보면 위장관으로 넘어간 혈액을 구토한 경우가 16예(11.9%)로 가장 많았고,지혈처치 당시는 출혈이 멈추었다가 이튿날 다시 출혈이 발생한 경우가 10예(7.4%) 있었으며,전신의 다른 부위 출혈과 연관된 저혈량성 쇼크,실신,흡인성 폐렴 등도 소수에서 발생되었고,뇌 기저부 골절 등에 의한 해면정맥동 손상과 사골혈관 손상으로 지혈이 안 된 경우도 있었다(Table 9).
지혈여부를 확인하기 위해 시행한 이학적 검사(전혈검사,혈소판 숫자검사, prothrombin time, partial thromboplastin time)에서 비정상 소견을 확인한 결과 혈소판 숫자는 26예(26.3%)에서 10만 이하였고, prothrombin time은 21예(23.6%), partial thromboplastin time은 19예(21.3%), I.N.R.은 20예(22.5%)에서 이상소견을 나타냈다(Table 7).
후속연구
이상을 바탕으로 구강내 과다출혈의 관리에는 치과(구강악안면외과)와 관련의학과(내과, 응급의학과 등)의 긴밀한 협조로 신속 정확한 지혈관리가 이루어져야 하며,개인 치과의원에서 발치 등의 소수술 시에는 봉합,배농술,전신질환의 사전확인 등이 중요함을 인식시킴이 긴요하리라 사료되었다.
이에 저자 등은 최근 5년간 구강내 과다출혈을 주소(chief complaint)로 연세대학교 원주의과대학 원주기독병원 응급실로 내원한 135명의 환자를 대상으로 출혈의 원인,손상혈관 부위, 출혈성 질환별 발생 빈도, 치과질환의 특성, 이학적 검사소견, 지혈처치 내용, 합병증 발생 등을 임상적으로 연구하여, 향후 구강내 과다출혈 환자의 신속 정확한 진료에 대비코자 한다.
