With dramatic transitions from a traditionally food-insecure to a food-sufficient society, over weight and/or obesity are an increasing health concern in Korea. In 2000, $35.9\%$ of Korean adults were over weight (BMI>25) with increasing trends. It is well known that obesity is highly correlated with chronic diseases, such as diabetes, hypertension, cardiovascular disease, cancer and high cholesterol. In 2003, the social cost of obesity is estimated at $529.5\~799.3$ billion won (direct cost) and at $1,200\~1,817$ billion won (including indirect cost). The share of the direct cost is estimated at $2.6\~3.9\%$ of total medical bill 20,742 billion won. These costs are underestimated and would surely be higher if the expenditures outside of the official medical insurance system of Korea were included. Based on the rapid increase of obesity rate among the Korean youth, it is crucial to develop and implement appropriate policies to curb the obesity epidemic.

제안 방법

특정 질병에 대한 비만의 인구기여율 (PAR)은 문헌마다 차이가 있다. Kenkel and Manninge匆 당뇨의 60%, 심장질환과 뇌졸중의 20%, 결장암, 전립선암, 유방암의 35% 를 인구기여율로 설정하였다. Colditz",는 심장질환 40%, 결장암 11% 등을 적용하였으며, Colditz祯는 당뇨 57%, 결장암 9.
사망원인통계는 사망신고서의 사망원인 기재 내용을 한국 표준질병사인분류 (KCD)에 의거 분류하여 작성되며, 원인 의 분류는 19개 장 103항목 (WHO에서 권고한 항목), 56 항목 (사인순위 선정을 위한 항목), 236항목(우리나라에서 많이 발생하는 사인을 위주로 한 항목)별로 구분하여 작성한다. 비만관련 질환으로 알려진 질환이 사망원인으로 기록된 항목을 추출하여 분석에 적용하였다.
질환을 치료하는 동안 상실된 시간은 입원의 경우는 입원일수, 외래는 외래방문에 소요되는 시간을 기준으로 하였다. 입원요양일수에 해당하는 기간 동안 일평균임금 (86.7천원)을 적용하고, 외래에 대해서는 왕복, 대기 및 치료시간으로 외래 건당 평균 소요시간那 54분에 시급(10,098원)을 비례 적용하였다 (Table 7).

대상 데이터

본고에서는 사망자료는 통계청의 사망원인통계, 입원 및 외래환자에 대한 진료 정보는 건강보험심사평가통계연보에서 추출하였다. 자료를 추출하는 과정에서 비만과 관련이 있는 질환은 기존 문헌을 참조하였으며, 분석에 포함된 질환 및 KCD 분류는 Table 4에 정리하였다.
본고의 분석에서 사용한 자료는 통계청의 사망원인통계 와 건강보험심사평가원의 건강보험심사평가통계佃를 기본으로 하고, 필요시 건강보험공단, 노동연구원의 자료를 참조하였다. 모든 자료는 2003년을 기준으로 하였다.
비만과 관련된 질환에 대한 입원 및 외래에 대한 자료는 건강보험심사평가통계연보(건강보험심사평가원)의 질환별 요양급여실적 자료를 활용하였다.

宜-顺 고혈압, 고콜레스테롤, 골관절염, 심혈관질환, 뇌졸증, 인슐린비의존성 당뇨, 담낭질환, 유방암, 결장암 등. 비만은 다른 여러 질병의 발생위험을 증가시키고 행동의 제약을 유발하는 경향이 있는데, 가령 BMI > 40인 사람은 표 준 BMI인 사람에 비해 2형당뇨, 담낭질환 등의 위험이 현저히 높으며, (2) BMI가 높은 그룹은 표준그룹에 비해 입원이나 외래진료, 약국 및 검사서비스 비용을 더 많이 부담하는 것으로 나타났다.跡 스웨덴의 비만 여성은 병가(sick leave) 확률이 1.
셋째, 공식의료체계 밖에서 이루어지는 비만관련 비용은 전혀 반영하지 않았다. 1998년 공식의료체계 외부에서 이루어진 비만관련 지출의 규모가 19,888억원이고, 이 가운데 비만인구에 의한 지출액은 7,917억원으로 잠정 추정된 바 있는데, 이 규모는 공식의료체계를 전제로 산출한 직간 접 비용 약 2,598억원의 3배를 넘는 방대한 금액이었다.
첫째, 병원과 약국을 포함 총의료비 가운데 (급여, 비급여) 본인부담금 수준이 43.2%, 외래의 63.7%로 추정된다이 비율에 따라 진료비에서 본인부담을 역산하면, 비만에 의한 비용은 직접비가 1,723억원 (PAR), 2,188억원 (PARo), 2,813억원 (PARQ씩 증가하며, 따라서 간접비용을 합산 한 총비용은 1.4~2.1조원에 이르게 된다.

후속연구

상대위험은 위험요인이 없는 집단에 비하여 위험요인을 가진 집단에서 특정 질환의 발생위험을 나타내며, Odd-ratio와 달리 횡단면 자료가 아닌 지속적인 추적연구를 통해 구해야 한다.
