제안 방법
4-hole miniplate와 4개의 miniscrew를 이용하여 골절부의 fixation을 시행하였고(Fig. 5) 공여부에는 지혈을 돕기 위해 fibrin adhesive(Greenplast®; Green Cross Co., Youngin, Korea)를 적용하였다.
본 교실에서는 연조직 손상을 방지하기 위해 piezoelectric surgery system(Piezosurgery®)를 사용하였다.
2). 상악 전치부의 치조골 폭을 증대시켜 임플란트 식립부의 골폭을 확보하고 골흡수로 인한 3급 골격 관계를 개선하고자 block bone graft를 계획하였다. 좌측 하악지의 block bone을 채득하기 위해 Piezosurgery®(Mectron, Genoa, Italy)를 이용하여 골절단을 시행하고(Fig.

임플란트로의 접근을 위해 상악동 측벽에 Piezosurgery®를 이용해 골창을 형성하고(Fig. 13) 상악동 막을 천공시켜 임플란트를 제거하였다(Fig. 14).
차후 임플란트 식립을 위해 동종골(Tutoplast Cancellous microchips®; Tutogen Medical GmbH, Germany)과 이종골(BBC®; Oscotec, Korea) 및 tetracycline (Chong Kun Dang Pharm., Seoul, Korea)을 혼합해 골이식한 뒤 흡수성 차단막(Tutoplast Pericardium®)을 적용하였다 (Fig. 16).

대상 데이터
47세 여자 환자로 상악 임플란트 식립을 위해 내원하였다. 약 4년 전부터 상, 하악 국소 의치를 사용했으며 상악 양측 제1대구치와 하악 전치부만 잔존해 있었다.
49세 남자환자로 좌측 안면부 종창을 주소로 응급실 내원하였다. 고혈압 외에 특별한 의과적 병력은 가지고 있지 않았다.
53세 여자 환자로 상악동 내로 전위된 임플란트를 제거하기 위해 개인 치과의원에서 의뢰되었다. 1년 전 개인 치과의원에서 상악 우측 제2소구치, 제1, 2대구치 발치 후 제2소구치, 제1대구치 부위에 임플란트 식립하였고, 2개월 후 제1대구치 임플란트가 탈락하였다.
