The purposes of this research are to survey work-related risk factors of musculoskeletal disorders(MSDs) in various departments and tasks at general hospitals in non-manufacturing sectors, and to use basic data derived from the survey results in preventing work-related MSDs in hospital workers. Investigation started in March of 2006 and continued for 6 months in 220 general hospitals at Seoul, Incheon, Kyeongi, and Kangwon area. Investigators visited and interviewed workers in hospitals to identify the presence of tasks of musculoskeletal burden, the investigation results of ergonomic risk factors required by the Occupational Safety and Health Law, statistical analysis from questionnaire for musculoskeletal symptoms, and major departments and tasks that have such risk factors. Twenty-seven percents of hospitals finished the investigation of ergonomic risk factors, and 69% did not do the investigation while remained 4% did not have such factors in their hospitals. The rank order of major departments that had such musculoskeletal burdens was kitchen rooms of 143, managing departments/computer rooms/dispensaries of 137, physical treatment rooms of 109, nursing departments of 96, radiological and clinical laboratories of 63. Eighteen hospitals that did not hold legal duties by the section 148 of labor minister decree practiced prevention programs of MSDs according to the labor-management cooperation. Nursing departments ranked in the first place for the numbers of musculoskeletal symptoms of 438. Managing departments/computer rooms/dispensaries, kitchen rooms, and medical treatment departments held 127, 52 and 45 symptoms, respectively. The magnitude order of physical symptom areas followed shoulder of 185, backs and waist of 166, hand wrists of 120, necks of 110, and legs/feet of 106. The departments and jobs that had major work-related ergonomic risks were patient transporting, central supplying, patient nursing (moving patients into wheel-chairs, changing of patient posture and sheet alteration), manual transporting, operation, and managing/computer departments.

문제 정의
본 연구는 근골격계질환이 조선 및 자동차업종 등 제조업종에서 다발하였으나 최근에 의료업종을 포함한 비제조업분야에서 점차 증가하고 있어 현장 실태조사를 통한 정확한 작업위험요인을 조사하고 공학적, 관리적 대책을 제시코자 하였다.

제안 방법
공학적 개선안으로 기존에 시트를 펴서 매트 밑으로 집어넣는 동작(그림 16 참조)개선을 위해 시트 모서리 부분을 침대에 맞게 개조하여(그림 17 좌측) 교체가 용이하도록 개선을 권고하였으며, 관리적 개선안으로 작업 시 3인 이상 작업, 단위작업별 적합한 스트레칭 실시를 권장하였다(그림 17 우측).
또한 배식공정의 경우 배식 카를 시야확보가 가능하도록 적정규격으로의 교체를 권장하였고 단문개방형을 양문 개방형으로 교체하여 손 뻗음 동작의 최소화를 권장하였으며, 식판 적재 시 무릎 아래 선반에는 가능한 적재 금지, 배식카 바퀴 직경은 가능한 큰 것으로 교체를 권장하였다(그림 26). 관리적 개선안으로 중량물 적재 시 권장 적재구간(무릎 ~어깨)제시, 식자재 창고 등 주요 중량물 취급장소 내 산업 보건 기준에 관한 규칙 제 151조 중량물 특별조치에 의한 안내표시 부착 및 교육, 단위작업별 적합한 스트레칭 실시를 권장하였다.

대상 데이터
소매 등 유통업종, 음식 및 숙박업종을 대상으로 실시하였으며, 본 연구는 그중에 의료업종에 대한 실태조사의 결과를 정리한 것으로 결과를 살펴보면 다음과 같다.
실태조사는 2006년 3월부터 8월까지 6개월간 한국산업안전공단 서울지역본부 권역 내(서울 경기, 인천, 강원지역) 220개 병 .의원급을 선정하여 현장 방문을 통해 작업 특성을 파악하기 위한 면담 조사와 촬영 등을 실시하였고, 인간공학적 정밀평가도구를 이용한 분석 과정을 거쳐 개선안을 도출하였다(표 1).

넷째, 산업안전 보건에 대한 전반적 인식이 부족하였으며 보건관리조직 및 체계, 보건관리 전담인력배치가 미흡하였다. 다섯째, 근골격계질환 증상호소자에 대한 사후관리, 재활, 치료 등에 대한 인식과 노력이 부족하였다.

또한 의료기관 내 각 부서별 근골격계부담작업 보유실태를 조사한 결과 전체 220개 조사사업장 중에 식당부서가 143개소로 가장 많은 근골격계 부담작업 보유부서로 파악되었으며 원무(전산, 접수 포함) 및 약국 부서가 137개소, 물리치료실이 109개소, 간호부가 96개소, 방사선 및 임상병리실이 63개소 순으로 나타났다(그림 2).
또한, 근골격계 질환자 22명에 대한 발생분포를 근속기간별(그림 7) 및 요양기간별(그림 8)로 살펴보면, 2년 미만 종사자가 전체의 55%, 1년 미만의 요양을 보낸 자가 전체의 95%로 나타났으며, 특히 사고 성 무리 동작으로 인한 근골격계 질환자의 요양기간은 평균 65.64일로 나타나 신체 부담(78.6일), 비사고성 작업 관련 요통(79.7일)보다 비교적 짧았다.
셋째, 병원은 전통적으로 경영 방향이 건강유지보다 질병 치료에 우선되어 병원 종사자의 의료욕구는 상대적으로 간과되기 쉬웠다.
조사사업장 220개소 중 2003년도 근골격계질환 예방 관련법 제정 이후 최초 유해요인조사를 실시한 사업장은 59개소(27%)였으며, 최초 유해요인조사를 미실시한 사업장은 152개소(69%), 근골격계부담작업이 없어 해당 없는 사업장은 9개소(4%)로 나타났다(그림 1).
조사사업장 중 증상 설문조사를 실시한 사업장에 대한 통계분석 결과, 부서별 통증 호소 분포로 보면 간호부서가 438 건으로 대부분을 차지하고 있었으며 원무/전산/약국부서가 127건, 식당부서 52건, 진료부가 45건 순으로 나타났으며 (그림 4), 이를 신체 부위별 통증 호소 분포로 보면, 어깨 부위가 185건, 허리 166건, 손 부위 120건, 목 110건, 다리/발 106건 순으로 나타나 간호부 종사 근로자의 대부분이 어깨, 허리, 손 부위, 목, 다리 등에 통증을 호소하고 있는 것으로 파악되었다(그림 5).
첫째, 의료업종은 다양한 부서(직종) 및 전문가들로 구성되어 각 분야별 작업 전환이 매우 어려웠다.
표 3은 실태조사 사업장 220개소 중 전년도(2005년)에 발생한 근골격계질환자 발생실태 파악 결과 약 10%인 21 개소 22명이 발생하였으며 신체 과도한 부담 및 비사 고성 작업 등 작업 관련성 질환이 8개소(9명), 사고성 무리동작이 13개소(13명)이 발생하였다.
후속연구
따라서, 본 연구에서 제시한 작업위험요인 및 대책안에 대한 자료를 토대로 의료업종 종사자에 대한 근골격계질환 예방의 기초자료로 활용되길 기대한다.
