The following results on patient type, implant distribution and survival rate were compiled from 324 implant cases of 140 patients treated at the periodontal dept. of Yonsei University Hospital and G dental clinic between February 2003 and April 2006. 1. There are no dissimilarities between men and women, with patients in their 30, 40, 50s accounting for 80% of patients and accounted for 82% of implant treatments; the largest share of patients and implant treatments. 2. The major cause of tooth loss is periodontal disease, followed by dental canes, trauma and congenital missing. 5, The distribution of bone quality for maxillae was 54,2% for type III, followed by 30.8% for type II, 15% for type IV and 0% for type I. As for mandible, the distribution was 63% for type II, followed by 34% for type III, 2,5% for type I and 0,5% for type IV. 6. 제안 방법
직경 5) 가용 골의 제한에 따른 복잡 술식 6) 치료된 보철물의 종류 등에 대해서 조사하였다
이때, 조절되지 않는절대적인 금기증의 환자들은 임플란트 수술을 시행하지 않고, 다른 방법을 이용하여 상실된 치아를 수복하도록 하였다. 또한 문진을 통해 환자의 발치원인 및 시기 등을 조사하였다.
모든 환자들에 대해 구강 검사 및 방사선 검사를시행하였으며 당뇨병, 방사선 치료 유무, 혈액 질환, 정신 질환 등에 대한 전신 병력 검사와 함께 흡연에대한 검사도 함께 시행하였다. 이때, 조절되지 않는절대적인 금기증의 환자들은 임플란트 수술을 시행하지 않고, 다른 방법을 이용하여 상실된 치아를 수복하도록 하였다.
보철물의 유형은 부분 무치악에서 단 일치 수복과 2개 이상의 임플란트를 이용한 고정성 브릿지 형태로 나누었고 완전 무치악에서는 임플란트와 주위 조직으로부터 지지를 받는 보철물과 임플란트만 지지를 받는 보철물이 제작되었다.
수술 부위의 골 상태는 Lekholm과 Zarb의 분류에1°)따라, 골질 및 골 양을 수술 시에 평가하여 기록하였으며, 수술에 사용된 임플란트는 길이, 직경에대한 기록을 통해 평가하였다.
임플란트 수술을 받은 환자의 치아 상실 원인을 충치, 치주염, 선천적 결손, 외상으로 인한 치근 파절, 기타 등의 5가지 분류를 이용하여 조사하였다. 충치의 분류는 보존수복이 불가능하여 발치한 경우 및 신경치료와 연관되어 치아를 상실한 경우를 포함한다.
총 324개의 임플란트 수술 부위 중 161개의 경우에서 골양의 부족을 이유로 임플란트 이식 외에 골이식, 인공 차폐막의 사용, 그리고 상악에 있어서 상악동거상술을 필요로 하였다

대상 데이터
2003년 2월부터 2006년 4월 사이에 연세 대학교 치과 대학병원 치주과와 人치과에서 임플란트 수술을 받은 환자 중 Xive® 임플란트 시스템(Xive® im plant, Frialit®, Friadent, Dentsply)을 이용하여 시술받은 140명의 환자에게 이식된 324개의 임플란트를 대상으로 하였다. 각 항목별로 검색이 가능한경우에 통계에 포함시킬 수 있었다.
골 이식을 동반한 골 유도 재생 술을 시행한 126개의 경우에 있어서 자가 골만을 단독으로 사용한 경우는 25개이며 나머지 101개의 경우에는 자가골과 합성 골을 혼합하여 사용하거나 혹은 합성 골 단독으로 사용하였다(Table 9).

사용된 임플란트의 길이는 11mm 이상이 80% 사용되었고, 임플란트의 직경은 표준 임플란트인 4.5mm가 가장 많았으며(51%) 다음으로는 3.8mm (31%)가 많았다. 이는 임플란트 식립 부의 가용골 협설측 두께와 인접 치아와의 관계에 의해 제한되기 때문이다 8, 19).
식립 된 324개의 임플란트 중 288개의 임플란트를이용해 단일치 형태와 고정성 브릿지 형태 등 총 189 개의 보철물이 제작되었다.
이번 연구에서는 324개의 임플란트가 식립되면서골양의 부족을 개선시키기 위해 161개의 임플란트식립시 임플란트 이식 외에 자가 골 및 합성골 이식, 인공 차폐막이 사용되었고, 이 중 상악 구치부 임플란트 식립을 위해 35case 에서는 상악동거상술을 필요로 하였다
성능/효과
1. 환자의 유형에서 남녀 차이는 없으며, 30〜50 대에서 환자 수로는 80%, 임플란트 수는 82% 로 가장 높은 분포를 보였다
2. 골질은 상악에서 iype 111(54.2%), 11(30.8%), IV(15%), I (0%) 순이었고, 하악의 경우Type 11(63%), 1(34%), 1(2.5%), N(0.5%) 순의 분포를 나타내었다

30대와 40대, 50대의 환자 수가 각각 37명, 37명, 38명으로 비슷하였으나, 매식된 임플란트의 개수는 63개, 91개, 114개로 한 명의 환자에게 매식된 임플란트의 개수가 30대는 평균 1.7개, 40대는 평균 2.5 개, 50대는 평균 3개로 연령의 증가와 더불어 매식되는 환자당 식립 되는 임플란트의 수가 증가함을 볼수 있었다.
5.전체 식립된 Xive® 임플란트의 생존율은 98% 였다.상악에서는 97%의 생존율을 나타내었고(식립된 120개의 임플란트 중 4개 제거), 하악에서는 99%의 생존율을 나타내었다 (식립된 204개의 임플란트 중 2개 제거).
임플란트의 수도 남자보다 더 많았다. 또한 수술받은 환자의 수는 30대, 40대, 50대가 비슷하였으나연령의 증가와 더불어 환자당 식립되는 임플란트의수가 증가하여, 전체 평균(환자당 2.3개)에 비해 40 대평균 2.5개)와 50대(평균 3개)가 높았으며, 30세이하의 연령층에서는 환자당 평균 1.3개로 단일치수복이나 2개 이하의 임플란트가 식립 되었다
상악에서는 총 120개의 임플란트가 식립되었고 이중 4개가 제거되어 97% 생존율을 나타냈고 하악에서는 총 204개의 임플란트가 식립 되었고 이 중 2개가 제거되어 99%의 생존율을 나타내었다 상악에 있어서하악보다 낮은 생존율을 보이는 것은 치아상실 경과 기간에 따라 실제로 골이 흡수되는 정도의 차이와 함께 상악골 양 자체가 하악에 비해 적고 같은 흡수가 일어나도 남게 되는 골이 적기 때문이며, 하악에 비해 낮은 골질을 보이기 때문이라고 생각된다與).
이번 연구에서 Xive® 임플란트의 생존율은 98%로나타났다.
이상의 결과를 통해 Xive® 임플란트는 다른 여타임플란트 시스템과 비교하여도 안정적인 임플란트로선택하여 사용되어질 수 있음을 알 수 있었고, 골질과 골양의 제한이 있는 부위에서는 부가적인 술식을통해 골질과 골양의 개선을 고려하여야 한다.
임플란트 식립 부위의 골질에 따른 생존율을 조사한 결과 Type I 골에서는 83%(6개 중 1개 제거), Type II골은 99%, Type HI 골은 97%, Type 田골에서는 100%의 성공률을 나타내었다{Table 12).
조사 결과는 치주염(50%), 충치(40%), 외상(3%), 선천적 결손(3%)의 순서로 나타났고, 기타는 4%였다(Table 6).
3개로 비슷하였다. 총 140명의 환자에게 324개의 임플란트가 매식되어 한 명의 환자에게 평균 2.3개의 임플란트가 수술되었다.
총 324개의 임플란트 중 204개가 하악에 위치되었으며(63%), 주로 좌, 우측 대구치 부위에 집중적으로 매식되었다(전체 하악 임플란트의 72%).
최근의 연구 결과에 의하면 낮은 골밀도를 지닌부위에서 치근형 임플란트를 사용함으로써 식립 시에 측벽에 대한 압축 효고 K lateral condensation)0]] 의해 골밀도가 개선됨을 확인 할 수 있었고, 골내 수용 부와 임플란트 간의 긴밀한 접촉이 시술 초기에 얻어짐을 확인 할 수 있었다. 많은 연구를 통하여 기계적인 절삭에 의해 형성된 매끈한 표면을 가진 임플란트에 비해서 거친 표면의 임플란트가 골질이 낮은 부위에서 더 성공적으로 사용될 수 있음이 보고되었다 3).
