Methods : This study investigated 264 community mental health professionals and 228 hospital workers. SOQs (Suicidal Opinion Questionnaires) were used from July 2007 to September 2007. After a factor analysis for the attitude towards suicide, the items on ethics, mental illness, religion, risk, and motivation were included in the subsequent analysis. Results : There were significant differences in the attitude towards suicide according to religion, age, educational background, the marriage status, the economic position, and different professional licenses. Hospital workers' view was different from the community workers'. The hospital workers judged that suicide was due to mental illness, and suicide was high for the people in a special environment and who lacked motivation, which caused them to fall in a dangerous situation. For the lower educational group, they thought that suicide was attributable to mental illness. The awareness for suicide was significantly higher in the group with a postgraduate education, unmarried people, mental health professionals and the persons who had concern and experience with suicide. The factors that had an influence on the awareness of suicide were the items of mental illness, religion, risk and motivational factors. Conclusions : This study suggested that the factors to increase the awareness and attitude for suicide were the experience of increased education and case management of suicide. Therefore, education dealing with suicide and reinforcement of crisis management programs should be developed.

연구 대상자들의 일반적인 특성을 알아보기 위하여 성별, 연령, 학력, 결혼상태, 종교, 정신보건요원 자격증 여부, 직위, 근무경력 등으로 문항을 구성하였다. 연령은 20대, 30대 이상으로 학력은 대학졸업, 대학원 재학 이상으로 나누었다.
연구 대상자들의 자살에 대한 경험적 특성을 5문항으로 구성하였다. 자살관련 서비스 제공에 관심이 있는가', 가까운 친지나 가족의 자살경험이 있는가', '자살관련 사례관리 경험이 있는가', 자살 사례에 대한 수퍼비전(자문)을 받은 경험이 있는가', 자살관련 교육을 받은 경험이 있는가' 에 대해서는 그렇다', '그렇지 않다'로 반응하도록 본 연구자가 직접 구성하였다.
대상 데이터
2007년 7월 1일부터 9월 3일까지 전화를 이용하여 팀장 및 수간호사들의 동의를 얻은 후 우편으로 설문지를 발송하였고, 다시 회수하여 응답이 부족하거나 대상이 적합하지 않은 13부를 제외한 지역사회근무자 264부와 병원근무자 설문지 228부인 492부를 분석에 이용되었다. 이 연구는 가톨릭대학교 생명윤리심의위원회에 승인을 받았다.
본 연구의 지역사회 정신보건근무자는 전국 모델형 정신보건센터 75개소와 알코올 상담 센터 26개소 중 전화로 동의한 정신보건센터 40개소와 알코올 상담센터 6개소에 근무하고 있는 280명을 대상으로 하였으며, 병원근무자는 서울·경기 지역에 소재하고 있는 대학병원, 종합병원, 시립병원, 정신과 전문병원 등 8개소에 전화로 동의한 병원근무자 240명을 대상으로 총 520명을 연구대상으로 하였다.
데이터처리
0 통계프로그램을 이용하여 연구 대상자들의 인구 사회학적 특성과 자살에 대한 경험적 특성, 자살태도 및 자살에 대한 인식수준을 알아보기 위하여 빈도 분석, x2-test, t-test를 사용하였으며 자살태도의 하부요인들을 추출하기 위하여 요인분석을 실시하였다. 또한 자살태도 및 자살인식 수준의 관련성을 알아보기 위해 다중회귀분석을 사용하였다.
수집된 자료는 SAS 8.0 통계프로그램을 이용하여 연구 대상자들의 인구 사회학적 특성과 자살에 대한 경험적 특성, 자살태도 및 자살에 대한 인식수준을 알아보기 위하여 빈도 분석, x2-test, t-test를 사용하였으며 자살태도의 하부요인들을 추출하기 위하여 요인분석을 실시하였다. 또한 자살태도 및 자살인식 수준의 관련성을 알아보기 위해 다중회귀분석을 사용하였다.
이론/모형
Domino 등 [27]이 개발한 자살에 관한 자기보고형 설문지인 Suicide Opinion Questionnaire(SOO) 중에서 우리나라의 문화적 상황에 적절하다고 생각되는 58개의 문항을 추려서 번역한 검사지를 사용하였다 [16]. 이 검사지는 “매우 그렇다"(2), “그렇다"(1), “잘 모르겠다"(0), “그렇지 않다"(-1), "전혀 그렇지 않다"(-2)로 구분되는 5점 척도로 반응하도록 구성되어 있다.
성능/효과
“자살은 종교적 신념이 없어서 나타난다"(요인3; religion)고 보는 요인에서는 두 군 모두 종교가 있는 군에서 유의하게 높게 나타났고, 지역사회근무자에서 기혼일 경우, 병원근무자에서는 자격증이 없는 경우 자격증이 있는 사람에 비해 유의하게 높게 나타났다.
지역사회근무자에서는 동기 요인에서는 자살에 대한 상담이나 사례 유무에 따라 차이를 보였다. 병원근무자의 윤리적 요인에서 자살관련 서비스 관심이 있는 경우, 가까운 사람의 자살경험유무에 따라 유의한 차이를 보였고, 정신질환요인에서는 자살관련 서비스 관심 유무에서 유의한 차이를 보였고, 종교요인에서는 가까운 사람의 자살 경험 유무에 따라 유의한 차이를 보였다(Table 5).
본 연구결과 지역사회근무자에 비해 병원근무자에서 자살이 정신질환에 기인하며, 위험한 상황에 놓인 특수한 환경과 동기를 가진 경우 더 자살을 하게 된다고 하였다. 이는 Park 등 [10]의 연구에서 개원의사와 일반병원의사들은 자살을 정신질환에 의한 것으로 보는 경향이 높게 나타난 연구와 일치하였다.
교육수준에 따른 자살태도 및 인식수준에 대한 연구가 국내에서는 없었으나, Singh 등 [33]의 연구에 의하면 교육수준이 높을수록 자살에 더 허용적인 태도와 높은 인식수준을 보인다고 하여 본 연구와 일치하였다. 본 연구에서 지역사회 근무자, 학력이 대학원이상, 교육을 받은 경우, 전문요원의 자격증을 가진 경우, 자살관련 경험이 있는 경우에서 자살에 대한 인식수준이 높게 나타났다. 이것은 Kim [34]의 보고서에서 전문가의 자살에 대한 지식과 윤리적 규범을 강조하기 위해 주요 국가에서는 자살예방을 위한 다양한 노력을 하고 있다고 하였으며, Quinnett [35]은 자살로 대상자를 잃은 경험이 있다면 자문이나 지지를 구하거나 필요시 상담을 받아 볼 것을 권하고 있다고 하였다.
이상의 연구결과로 자살 태도에 있어서 교육 및 사례관리 경험이 있을 때 높은 인식수준을 보였다. 향후 연구대상자들의 자살에 대한 바른 인식과 위기 상황의 대처능력을 강화하는 교육들이 진행되어야 할 것이며, 더불어 자살자가 있는 가족들의 사후개입을 위한 교육 프로그램을 개발하는 연구들도 지속적으로 이루어져야 할 것이다.
자살에 대한 인식수준에 영향을 주는 요인으로 지역사회 근무자와 병원근무자 두 군 모두에서 자살태도 중 윤리, 정신질환, 종교, 위험, 동기 항목이 인식 수준에 유의하게 영향을 주는 것으로 나타났다(Table 7).
지역사회 근무자와 병원 근무자의 자살 태도는 1요인(ethics)을 제외하고 모두 유의한 차이를 보였으며, 인식수준은 지역사회의 근무자가 유의하게 높았다.(Table 3).
지역사회 근무자와 병원근무자 모두 가족 및 친지 등의 가까운 사람의 자살에 대한 경험에서는 유의한 차이가 없었으나, 자살에 대한 관심, 사례경험, 자살사례에 대한 수퍼비전을 받은 경험, 자살관련 교육 경험에서는 모두 병원근무자 보다 지역사회 근무자가 더 높게 나타났다(Table 2).
