When the patients were asked about the convenience of the equipments, 15 people(50%) answered CR is convenient and 14 people(46.7%) answered the Tilting detector is convenient, showing not much difference. However, when the inspectors were asked the same question, 4people(80%) out of 5 answered that the Tilting detector is more convenient The time test showed that CR takes 2 minutes and 50 seconds, the Fixed detector 1minute and 48 seconds andor had no distortion showing the shoulder joint space. 따라서 본 연구에서는 디지털 환경에서의 견관절 Su- peroinferior axial projection 검사 시 CR(Computed radiography), Fixed-DR(digital detector radiography), Ti- lting-DR(digital detector radiography) 에 따른 검사 자세의 변화에 따라 편의성, 소요시간 및 영상의 질을 비교 평가하여 임상적용에 도움을 주고자 하였다.
제안 방법
검사자세에서부터 이미지가 PACS에 재현되는 시간까지를 측정하여 신속성을 평가하였다(Table 4).
환자를 10명씩 3그룹으로 나누어 장비별로 영상을 획득하였다. 획득한 각각의 영상을 근골격계 영상의학과 교수, 정형외과 전공의, 5년 이상 숙련된 방사선사 3명에게 블라인드 테스트를 실시하여 영상을 평가하였다.
환자의 협조를 얻어 CR, Fixed-DR, Tiltie-DR을 이용하여 한 장비로만 검사하였고 나머지 두 장비에서는 자세만을 취하였다(Fig. 1). 검사를 한 방사선사 5명과 검사한 환자 30명을 대상으로 설문 조사하여 자세의 편의성을 평가하였다(Table 2~3, Fig.

대상 데이터

본 연구에 사용된 Tiltable standing detector 장비는 Philips Cesium iodide scintillator, amorphous silicon Flat Panel Sensor로 구성된 Detector를 사용한다. 기존의 Detector는 Table or Standing type 모두 고정형 이었으나 연구에 사용된 장비는 Standing Detector가 전후방-20°~+90° tilting이 가능하다.
본원에 내원하여 shoulder axial 검사 처방이 있는 환자 중에서 외전이 90° 이상 가능한 30명(2008년 11월~ 2009년 1월)을 대상으로 하였다. 조사 대상의 일반적인 특성으로 연령분포 는 50세 이상(33.

CR system과 Fixed-DR, Tilting-DR system에서의 Shoulder axial projection image를 숙련된 방사선사와 영상의학과 전문의, 정형외과 전공의가 평가한 결과 CR image와 Tilting-DR image는 임상적용에 문제가 없는 영상 이였으나 Fixed-DRe 임상에 적용하기에 질이 떨어지는 영상으로 판정하였다. 그 중에서도 Tilting-DR 영상이 높은 점수를 받았다.

검사자세의 편의성에서는 환자, 검사자 모두 Tilting-DR을 가장 선호하였다. Fixed-DRe 팔을 많이 외전 시켜야 하고 머리와의 겹침으로 인해 자세 또한 불안정하여 제일 낮은 선호도를 보였으며, CR에서는 비교적 자세잡기가 수월하였으나 몸을 IP cassetbe쪽으로 많이 기울여야 하는 단점이 있었다. 그에 비해 Tilting-DRe 앉은 자세에서 팔만 벌려 자세를 취하므로 크게 문제되지 않았다.
각 장비별로 검사시간을 측정하여 평균을 얻은 결과 CRe 2분 59초, Fixed-DRe 1분 48초, Tiltin#은 1 분 43초로 조사되었다. 또한 20세 이하에서는 CR과 Tilting-DR에서 1분 28초로 가장 많은 차이를 보였으며 40세 이상에서는 CR이 3분을 넘는 시간이 소요되기도 하였다(Fig.
그에 비해 Tilting-DRe 앉은 자세에서 팔만 벌려 자세를 취하므로 크게 문제되지 않았다. 검사자세에서 이미지 재현까지의 시간을 알아보는 검사 소요시간에서는 TUting- DR과 Fixed-DR에서 1분 30초 정도로 CR보다 2~ 2.5배 정도 빠른 것으로 나타났으며 재촬영에서도 TUting-DR 과 Fixed-DRe 20~30초 사이에 이루어져 image를 얻는 반면 CR에서는 CR Reader기를 통해 image가 재현되는데 2~2.5분이 소요되는 것으로 나타났다.
