문제 정의

제안 방법
⑤ 경혈이 조합을 이루어 선정된 경우를 알아보기 위해, 다른 경혈과 함께 사용된 빈도를 총 사용빈도로 나누어 조합율을 계산하였다.
논문을 선택하는 데 있어서 국내 논문의 경우 검색된 학위 논문을 포함시켰으며 동일한 연구가 학술지에 기재된 경우 내용 확인 후 학술지를 기준으로 정리하였다. 침 치료의 경우 피부를 뚫는 침을 이용한 경우를 포함시키고 레이저 침, 경혈마사지 등과 특수한 경혈을 이용하는 이침, 수지침 등을 제외하였다.
이번 연구에서는 국내 연구와 해외 연구에서 보고된 결과 지표에서도 약간의 차이가 나타났는데, 국내 연구의 경우 VAS를 보고한 비율이 해외 연구에 비해 높았으며(국내 12/17, 해외 6/20) 해외 연구의 경우에는 더 많은 종류의 결과 지표를 보고한 것을 볼 수 있었다(국내 23, 해외 40). 본 연구에서는 가장 많이 사용된 결과 지표인 요통 VAS 값을 기반으로 하여 치료 전후의 통증개선을 치료 결과로 적용하여 각 경혈별 치료효과를 비교하였다. 경혈의 효과를 알아보기 위해 치료 전후의 통증 정보 비교를 침의 효과로 정의한 것은, 침 치료와 다른 치료법을 비교하여 효과가 있음을 증명하고자 한 것이 아니라 침 치료로 인한 환자의 증상 변화에 경혈이 얼만큼 기여하는지를 알아보는 것이 목적이기 때문이다.

대상 데이터
포함된 해외 논문에 대한 내용은 다음과 같다(Table 2). 20편의 해외 RCT 9편, CCT 3편, Case report/series 8편이 포함됐으며, 이 중 28개의 연구가 분석되었다. 환자로는 병력 기간을 기준으로 포함시킨 환자, 비특이적 요통 환자, 특정 질환 환자, 임신 여성 등이 포함됐으며 결과 지표는 증상의 호전을 기술하거나 단계를 나눠 보고한 것 9편, VAS 7편, ODI 3편, McGill pain questionnaire(MPQ) 2편, RMDQ 1편 등이 사용되었다.
국내 검색엔진에 누락될 수 있는 문헌을 추가적으로 포함시키기 위하여 ≪대한한의학회지≫, ≪대한경락경혈학회지≫, ≪대한침구학회지≫, ≪한방재활의학과학회지≫, 및 ≪척추신경추나의학회지≫에서 ‘요통’으로 검색하였다(2012. 5. 30).
국내 논문 검색을 위하여 1차적으로 학술연구정보서비스[RISS(http://www.riss.kr)]와 한국학술정보[KISS(http://kiss.kstudy.com)] 전자 검색을 이용하여 ‘침’과 ‘요통’의 검색어를 사용하여 검색하였다(2012. 5. 24).
국내 요통 연구에서 경혈이 조합되어 사용된 비율을 나타낸 결과, 신수(BL23)와 함께 사용된 경혈은 기해수, 대장수(BL24, BL25: 71%); 관원수, (BL23)는 대장수(BL25: 77%); 양릉천(GB34; 위중(BL26, BL40: 64%); 그리고 환도, 곤륜(GB30, BL60: 50%) 순으로 나타났다. 기해수(BL24)는 대장수(BL25: 100%); 신수(BL23: 83%); 환도(GB30: 75%); 위중(BL40: 67%)의 순서로, 대장수(BL25)는 기해수(BL24: 100%); 신수(BL23: 83%); 환도(GB30: 75%); 위중(BL40: 67%)의 순서로 함께 사용되었다. 요양관(GV3)의 경우 신수(BL23), 기해수(BL24), 대장수(BL25), 환도(GB30) 와 항상 함께 사용됐지만(100%), 이 경혈들이 요양관과 함께 사용된 빈도는 50% 이하였다.
이 중 침, 요통에 관한 것이 아닌 논문이 94편, 영어가 아닌 논문이 51편, systematic review가 43편, RCT, CCT 혹은 Case report/series가 아닌 것이 99편, 치료군과 대조군의 치료 조건이 포함 조건에 맞지 않는 논문이 47편, 침을 놓은 위치에 대한 기술이 없는 논문이 12편이었다. 또한 포함된 논문의 대부분을 원문을 찾을 수 없는 1편의 systematic review에 포함된 논문 67편(중국 저널)을 제외하였다.
상기 방법으로 검색한 결과 332편의 논문이 검색되었으며, 수작업으로 102편의 논문을 더 찾았다. 중복을 제외한 후 제목과 초록을 확인했을 때 포함된 논문은 126편이었으며, 원문을 확인하고 최종적으로 포함된 논문은 20편이다.

총 227편의 논문이 검색되었으며 이 가운데 중복 검색된 논문을 제외한 후 제목과 초록을 확인했을 때 37편의 논문이 포함됐으며, 원문을 모두 확인하여 최종적으로 17편의 논문이 포함되었다. 제외된 논문 중 침, 요통과 관련된 것이 아닌 논문이 118편, SR논문이 3편, RCT, CCT 혹은 Case report/series가 아닌 논문이 21편, 적절한 치료 중재가 아닌 논문이 27편이었다(Fig.

에서 조사한 요통에 대한 경락 변증의 결과에서도 높은 빈도로 조사되었다. 또한 근위 취혈법(BL23, BL24, BL25)과 원위 취혈법(BL40, BL60, GB30)이 모두 이용되며, 아시혈을 이용하는 경우도 많다는 것을 알 수 있었다. 특히 해외 연구에서는 비경혈점과 아시혈이 많은 빈도를 차지하고 있는데, 이들은 하나의 경혈을 지칭하는 것이 아니어서 분석에서 제외하였다.

후속연구
결국 질환과 환자의 특성에 따라 경혈의 효과와 침 치료 효과를 나타내기 위해 적합한 결과 지표가 무엇인지, 그리고 같은 결과 지표를 보고하는 방식(Improvement rate, improvement degree, Mean±Standard deviation 등)에 관한 연구와 합의 과정이 필요하다.
본 연구에서의 제한점은 우선, 다양한 결과 지표중에서 VAS를 선택적으로 이용했다는 것이다. 다양한 결과 지표를 이용하거나 VAS 다음으로 많이 사용된 ODI를 이용해 같은 방식으로 평가해보았으나 치료 전후의 통계적 유의미한 차이를 알 수 있는 자료가 부족하여 의미 있는 결과를 도출하지 못했다.
그러므로 침의 효과를 알아보기 이전에 경혈의 정의, 특성, 특이적 효과에 대한 연구가 밑받침 되어야 한다. 앞으로 임상 연구와 진료 상황에서 경혈 선정과 같은 침 치료 전략의 구체적 사항에 대한 중요성을 인식하고 반영한다면, 연구 결과를 해석하고 진료 지침을 제시할 때 도움이 될 것이다.
그러나 경혈의 효과를 살피는 데는 논문에서 침 치료 방법에 대한 기술이 자세하지 않고, 단일 경혈이 아닌 조합된 형태의 경혈을 사용하는 점, 그리고 다양한 결과 지표를 이용해 서로 다른 방식으로 보고하는 점 등의 한계가 있었다. 앞으로 특정 질환에 있어 침 치료 전략을 세우고 경혈에 대한 이해를 돕기 위해서 각 질환별 특성에 맞는 경혈 선정을 고려한 연구가 진행되어야할 것이다.
원위 취혈 혈위들은 경락에 따라(족태양방광경：위중, 곤륜; 족소양담：환도, 양릉천) 조합되거나두 개의 경락(환도, 곤륜, 양릉천, 위중)이 조합을 이루기도 한다는 것을 알 수 있다. 이런 경혈의 조합은 통증 부위 및 환자의 증상을 경락 변증하거나 임상적 경험에 근거한 것으로 보이는데, 경혈의 조합을 이해하기 위해서는 임상 연구뿐 아니라 문헌적 고찰과 임상 진료 현장에 대한 설문 조사 등이 필요할 것이다.
