가돌리늄 조영제가 MR 검사의 진단학적 목적으로 널리 사용된 이유는? | 원자번호 64번의 금속성 물질인 가돌리늄은 자장에 민감하게 반응하는 상자성 물질로 독성이 매우 강한 특성을 지니고 있다. 이런 강한 독성 때문에 임상에서는 가돌리늄 원자에 리간드를 결합시킨 킬레이트 구조로 안정화하여 환자에게 투약 된다. 이렇게 안정화하여 투약된 GBCA는 불활성 상태로 자장 내 상자성 효과만 얻은 후 인체 내 신장을 통하여 배출된다. 이때 체내에 투약된 GBCA는 인체 내 H2O분자 중 H1 spin과 결합 하여 R1 relaxation time을 줄여주게 되는데, 이는 가돌리늄과 결합하지 않은 H1 spin에 비해 MR영상에서 높은 신호강도를 보여주게 된다[1~3]. 이러한 이유로 가돌리늄 조영제는 MR 검사의 진단학적 목적으로 널리 사용 되고 있다. |
