제안 방법

증례의 검사 방법은 환자를 바로 누운 자세에서 환자 자신에게 음경을 머리 쪽으로 당기도록 하고 수건을 말아 고환 밑에 고여 고환이 거상되도록 하였다. 양측 고환과 부고환을 회색조 영상(gray scale image)으로 장축과 단축을 얻고, 컬러 도플러 영상(color Doppler image)을 얻었다. 초음파 검사에서 고환과 부고환 실질의 에코 발생정도(echogenicity)를 살펴보고 고환의 크기를 측정하였다.

대상 데이터

건강검진 목적으로 내원 한 30세 남자이다. 내원당시 특별한 임상증상은 없었다.
결혼은 앞둔 예비 신혼 부부 건강검진 목적으로 내원한 29세 남자이다. 내원당시 특별한 임상증상은 없었다.
환자는 생식기 외상, 잠복고환, 성병 등은 없었다. 단지 15세에 심한 볼거리 고환염을 앓았던 경험이 있어 치료를 받았다. 혈압은 수축기 115mmHg, 이완기 75mmHg로 정상이었고, 비만도(BMI)는 25.
볼거리 고환염으로 정자 형태에 영향을 미치며, 정자운동성 감소, 정자감소증, 무정자증이 발생한다. 본 연구는 볼거리 고환염을 앓은 29세 및 30세 남성을 대상으로 정액, 혈청 호르몬 검사 및 고환 초음파 검사를 실시하였다. 정액 검사에서는 정자가 전혀 검출되지 않았다.

성능/효과

또한 Stratton[34] 등의 연구에서 볼거리 고환염을 앓은 49 중 39명이 다소 고환의 위축을 보였다. 본 연구에서 대조군과 볼거리고환염군의 고환체적은 11.8ml과 11.2ml으로 차이는 없었다. 그러나 무정자증을 보인 첫번째 증례에는 볼거리 고환염 후 고환의 체적은 우측이 1.
본 연구에서 대조그룹과 볼거리고환염군의 정자 수에 유의한 차이가 있으며(P<0.05), 볼거리 고환염군에서 대조군보다 정자감소증을 보여 다른 연구자들과 비슷한 결과를 얻었다.
6mIU/ml 이상이다[38]. 본 증례에서는 정세관 손상으로 정자는 발생하지 않지만 호르몬은 정상적으로 분비됨을 알 수 있었다. 볼거리 고환염의 대증적 치료는 고환의 얼음 팩 마사지, 침상안정, 고환거상 등이 있다[12].
정자 수는 대조군에서 8,177만(1500만/ml), 볼거리고환염군에서 2,637만(1500만/ml)으로 두 집단에서 유의한 차이를 보였으나(P<0.05) 볼거리 고환염군은 정자의 수가 정상 보다는 높게 나타났으나 대조군에 비해 낮아 정자감소증을 보였다.
