따라서 본 연구는 경로당을 방문한 60세 이상 여성 노인들을 대상으로 인지상태 검사, 즉 치매 검사와 구강검사를 동시에 실시하여 노인들의 치매와 잔존 치아 수 및 주관적인 저작 정도를 조사하여 서로의 관련성에 대하여 알아보고자 한다.

기능적인 면을 고려하여 잔존 치아의 개수를 0-10개까지 ‘거의 씹을 수 없음’, 11-20개까지 ‘겨우 씹을 수 있음’ 그리고 21개 이상 ‘잘 씹을 수 있음’으로 대상자를 구분하였다[7,13].
일반적 특성은 빈도분석을 하였고, MMSE 점수에 따라서 정상, 치매의심, 치매 군으로 나누어 교차분석을 하여 일반적인 특성을 조사하였다. 그리고 t-test와 ANOVA를 이용하여 주관적인 저작 정도 치아 수에 따른 특성들을 평균 비교하였다.

MMSE점수와 잔존 치아 수와 관련성을 평가하기 위하여 다변량 로지스틱 회귀분석을 실시하였으며 나이, 거주형태, 교육수준, 틀니사용 등을 보정하여 분석하였다.
그리고 t-test와 ANOVA를 이용하여 주관적인 저작 정도 치아 수에 따른 특성들을 평균 비교하였다.
수합된 모든 자료는 통계분석용 소프트웨어인 SPSS 22.0 프로그램(SPSS Inc, Chicago, IL, USA)을 이용하여 분석하였으며, 통계적 유의성 판정을 위한 유의수준은 5%로 설정하였다.
이론/모형
3점 ‘보통이다’, 4점 ‘어느정도 씹을 수 있음’, 5점 ‘잘 씹을 수 있음’으로 주관적인 저작 정도를 설문 조사하였다. 치매척도는 이미 신뢰성과 타당성이 검증되어 현재 많이 사용되고 있는 구조화된 설문 도구인 한국판 간이정신상태판별 검사(Mini-Mental Status Examination, MMSE-K)를 이용하여 조사하였다. 총 30점 만점으로, 선행 연구를 참고로 하여 24점 이상을 ‘정상’, 20-23점을 ‘치매 의심’, 19점 이하를 ‘치매’로 간주하였다[14,15].
성능/효과
그러나 본 연구 결과를 종합하면 MMSE 점수가 낮을수록 잔존 치아 수가 적을 위험비가 높게 나타났고 나이, 성별, 거주형태, 교육수준, 틀니 사용 유무를 보정하였을 때도 그 관련성이 나타났다.
그러나 치매의심인 대상자에 군에서 나이, 거주형태, 교육수준, 틀니사용을 보정한 결과 잔존 치아수가 0-10개로 적을 위험비는 5.27배 높은 것으로 나타났고 통계적으로도 유의한 차이가 있었다(p>0.05).
그리고 나이, 거주형태, 교육수준, 틀니 사용을 보정한 결과 ‘치매의심’군에서 치아가 11-20로 적을 위험비가 6.42배, 0-10개로 적을 위험비는 5.27배 높은 것으로 나타났고 통계적으로 유의한 차이가 있었다(p<0.05).

또, 주관적인 저작 정도에 따라서 틀니의 사용 유무를 살펴본 결과 통계적으로 유의한 차이가 나타났다(p<0.05).

본 연구에서도 MMSE 점수는 잔존 치아 수가 0-10개인 군은 22.30±4.43점, 11-20개인 군은 23.55±3.69점, 21개 이상인 군은 24.38±4.32점으로 잔존 치아수가 많을수록 MMSE 점수가 높은 것으로 나타났다.
