누진다초점 렌즈 착용의 자각적 만족감에 미치는 요인

목적: 누진다초점 렌즈 착용 시 자각적 만족감에 영향을 주는 요소를 평가하고 누진다초점 렌즈 처방에서 고려해야할 추가적인 검사항목을 제시하고자 하였다. 방법: 대상자는 안질환과 수직사위가 없으며, 조절력 3.00 D 이하($2.33{\pm}0.45D$)를 가진 노안이 있는 100명($54.30{\pm}7.02$세)을 대상으로 하였고, 누진다초점 렌즈 착용에 따른 자각적 만족감에 대한 설문조사를 실시하였다. 자각적 만족감에 대한 설문조사는 착용 후 걷기, 바닥 주시, 좌우 보기, 원근거리 선명도, 경사나 계단 오르내리기 등 총 5문항에 대해 평가하였다. 자각적 만족감에 영향을 주는 요인분석은 굴절이상도, 가입도, 나이 그리고 이향운동 용이성을 중심으로 조사하였다. 결과: 누진다초점 렌즈 착용 후 자각적 만족감에 영향을 주는 요인에서 굴절이상도는 유의성이 없었으며(p=0.47), 가입도, 나이와 이향운동 용이성은 유의하였다(각각 p=0.000, 0.048, 0.000). 특히 이향운동 용이성은 자각적 만족감과 높은 상관관계(r=0.773)를 갖고 있으며, 자각적 만족감의 설명변량도 $R^2=0.597$로 높았다. 결론: 이향운동 용이성이 누진 다초첨 렌즈 착용 후 자각적 만족감에 양의 상관관계와 높은 설명력을 보였다. 따라서 누진다초점 렌즈 착용의 만족감이나 예후 평가에 이를 활용한다면 누진다초점 렌즈 처방의 성공률을 높이는데 도움이 될 것이다.
