재입원률의 질 평가 지표로서의 정당성을 나타내는데 어려운 점은 무엇인가? | 재입원율이 질 평가 지표로서의 정당성을 갖기 위해서는 최초 또는 기준(index) 입원의 의료서비스가 적절했다면 재입원의 상당부분이 예방이 가능하다는 전제가 충족되어야 한다. 그러나 환자의 사회경제적 수준을 보정한 경우 의료기관별 재입원 변이의 48%가 감소하였고[9], 약 4건의 재입원 중 한 건 또는 그 이하 정도만이 피할 수 있는 재입원이었으며[10], 재입원 평가를 잘 받기 위해 중증도가 낮은 환자들을 선별적으로 입원시키거나 계획되지 않았던 재입원을 계획된 재입원으로 코드를 변경하는 의료기관의 행태가 보고되기도 했다[11]. 사회경제적 수준, 질병의 중증도, 기저질환, 동반질환, 그리고 환자의 선호도 등의 환자요인이 재입원에 미치는 영향이 크거나, 이러한 변수를 충분히 보정하지 못하는 경우에는 지표의 신뢰도와 타당성에 많은 논란이 있을 수밖에 없다[12,13]. 따라서, 이 연구에서는 위험도 보정 일반 재입원 지표를 활용하고 있는 미국, 영국, 캐나다의 현황과 재입원율 산출 시의 기준, 그리고 위험도 보정 방법을 살펴보고, 우리나라에서 재입원율의 정책적 활용에 있어 고려되어야 할 사항들에 대해 알아보고자 한다. |
