Purpose: This study attempts to examine the effects of a horticultural program on activities of daily living (ADL) and depression among older patients with mild dementia. Methods: This study adopted a quasi-experimental design-based nonequivalent control group pretest and posttest design, enrolling 30 older patients with mild dementia. The Korean Form of Geriatric Depression Scale was utilized, while the data was collected from the experimental group for 60 minutes, twice weekly for 6 weeks in 12 sessions. ADL and depression were assessed for both the experimental and the control group. Overall functions were assessed only for the experimental group. Results: The experimental group showed improvement in physical function, cognitive function, as well as psychological, emotional function and social function following each session (p<.001). The horticulture program was effective in both ADL ($Z^2=5.65$, p<.001) and depression (t=-5.24, p<.001). Conclusion: In this study, the horticultural therapy based on the Cox's interaction model had positive effects for older patients with mild dementia. Therefore, horticultural therapy may be commendably applied to older patients with mild dementia as a nursing intervention.

제안 방법

본 연구자는 Cox의 대상자 건강행위 상호작용 모델을 근거로 원예활동 프로그램을 개발하였다(Table 1). Cox의 상호작용 모델은 크게 ‘대상자 단독요소’, ‘대상자-전문가 상호작용요소’, ‘대상자 건강산출요소’로 구성되므로(Cox, 1986) 매회기 간호중재 프로그램에서 대상자-전문가 간의 상호작용을 통해 대상자의 일상생활수행능력 및 우울에 대한 긍정적 변화를 유도하고자 하였다(Figure 1).
연구대상자의 일상생활수행능력과 우울에 대한 사후 조사는 12회기의 프로그램을 모두 마친 직후에 2015년 10월 29일 실험군과 대조군을 대상으로 동일한 조사자가 실시하였으며 맹검법을 이용하였다.
원예활동 프로그램은 2015년 9월 21일부터 10월 29일까지 매주 2회(월, 목요일 PM 2~3시), 60분씩 6주간, 총 12회기 12시 간의 집단 프로그램으로 운영하였다. 프로그램은 실험집단인 B시 노인요양병원에 입원중인 경증치매 노인 15명 대상으로 병원 작업실에서 실시되었다.

대상 데이터

본 연구에서는 의료환경과 규모가 유사한 100병상 이상의 2개 병원을 선정하였으며 실험군은 B시에 소재한 노인요양병원, 대조군은 C시에 소재한 노인요양병원으로 하였다. 연구대상자 선정기준은 치매 환자 중 Mini-Mental State ExaminationKorea (MMSE-K)점수가 15~23점으로 나타난 65세 이상의 경증치매 환자로 담당의사와 수간호사로부터 매 회기 약 60분정도 진행 될 12회기의 프로그램에 참여가 가능하다고 판단된 노인 중 본 연구참여에 주치의가 승낙하고 대상자 및 보호자가 모두 동의한 자로 하였다.

데이터처리

수집된 자료는 SPSS/WIN for 18.0 프로그램을 이용하여 연구대상자의 일반적 특성은 실수와 백분율로 하였으며 실험군과 대조군의 동질성 검증은 x 2 test with Fisher's exact test, t-test로 분석하였다. 측정도구의 신뢰도는 Cronbach’s ⍺ coefficient로 검증하였으며 연구 변수의 정규성 검증은 ShapiroWilk test로 하였다.

우울은 Yesavage 등(1983)이 노인을 대상으로 개발한 자기 보고형 우울척도를 Jung, Kwak, Joe와 Lee (1997)가 표준화 한 한국형 노인우울검사(Korean From Geriatric Depression Scale, KGDS)로 측정하였다. 이 도구는 평상시 일어날 수 있는 문제들에 대해서 지난 1주 동안의 자신의 기분을 ‘예’, ‘아니오’ 로 응답하는 30개 문항으로 이루어져 있으며, 사고, 정서, 인지, 신체 및 사회적 측면을 골고루 반영하도록 하고 있어 전반적인 우울 현상을 평가할 수 있다.
일상생활수행능력은 Barthel과 Mahoney (1965)가 만성 질환자를 대상으로 개발한 Barthel Index를 Park, Cho와 Son (1995)이 수정 ․ 보완한 도구로 측정하였다. 이 도구는 식사하기, 목욕하기, 세면하기, 옷 입기, 배변조절, 배뇨조절, 화장실 사용, 의자/침대이동, 이동거리, 계단오르기 등 총 10문항으로 이루어져 있으며, 점수가 높을수록 기능상태가 좋은 것을 의미 한다.

성능/효과

본 연구는 원예활동 프로그램을 개발하여 경증치매 노인의 일상생활수행능력 및 우울에 미치는 효과를 검증한 비동등성 대조군을 이용한 유사 실험연구이다. 연구결과 본 연구에서 개발한 Cox의 상호작용 모델을 기반으로 한 원예활동 프로그램은 요양병원에 입원중인 경증치매노인의 일상생활수행능력 및 우울에 긍정적인 영향을 미치는 것으로 나타났다. 따라서 경증치매노인의 질병 진행을 예방하고 재활을 돕기 위해서 원예활동을 적용한 간호중재 프로그램을 임상 실무에서 적극적으로 활용해 보길 바란다.

후속연구

따라서 본 연구의 원예활동을 간호중재 프로그램으로 활용하여 경증치매노인의 일상생활수행능력을 향상시키고 우울을 줄일 수 있을 것으로 생각된다. 그러나 지역사회와 임상에서의 새로운 간호중재 방안으로 자리매김 하기 위해서는 표준화된 프로그램 개발을 위해 대상자를 확대하여 반복연구를 시도하고, 노인요양병원의 시설과 치료환경에 응용할 수 있는 다양한 원예활동 프로그램을 개발할 필요가 있다.
연구결과 본 연구에서 개발한 Cox의 상호작용 모델을 기반으로 한 원예활동 프로그램은 요양병원에 입원중인 경증치매노인의 일상생활수행능력 및 우울에 긍정적인 영향을 미치는 것으로 나타났다. 따라서 경증치매노인의 질병 진행을 예방하고 재활을 돕기 위해서 원예활동을 적용한 간호중재 프로그램을 임상 실무에서 적극적으로 활용해 보길 바란다. 그러나 본 연구의 결과는 간호 분야에서는 아직 기초 연구이므로 추후 일반화가 가능한 충분한 연구대상자를 확보하여 모수검증을 실시하고, 간호중재의 결과를 포괄적으로 평가할 수 있는 다양한 연구변수를 선정하여 측정하는 반복 연구를 시도해 보길 제언한다.
